BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - BOLTS AND NUTS FAIL TO MEET ASTM SPECIFICATIONS - BLRD-50-438/81-64, BLRD-50-439/81 FOURTH INTERIM REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. V. Crlenjak on September 24, 1981 in accordance with 10 CFR 50.55(e) as NCR's 1597 and 1598. This was followed by our interim reports dated October 22 and December 8, 1981 and February 19, 1982. Enclosed is our fourth interim report. We expect to submit our next report by September 28, 1982. We consider 10 CFR Part 21 applicable to this deficiency.

FOURTH INTERIM REPORT Description of Deficiency.
10,000, 3/4" stainless steel nuts (furnished by Chattanooga Bolt and Screw, Chattanooga, Tennessee) and 10,600, 3/4" x 3-1/2" stainless steel bolts (furnished by Challenge Enterprises, Chattanooga, Tennessee) were received not marked in accordance with ASTM specifications. Subsequent testing of the subject material by Singleton Materials Engineering Laboratory revealed the material failed to meet ASTM specifications. The associated documentation received with the material erroneously shows that the nuts and bolts do meet ASTM specifications. Approximately 450 of these bolts ana 1300 nuts have been issued to the field for installation.
Nonconformance report (NCR) 1597 pertains to the bolts and NCR 1598 pertains to the nuts.
Interim Progress TVA is continuing its evaluation of the bolts and nuts as specified in the last report. Numerous bolts and nuts cannot be accounted for at this time. However, TVA is continuing its search for them.
Also, test data produced by Singleton Materials Laboratory is being further evaluated by TVA.
