Appendix NOTICE OF VIOLATION Scott Radiological Group License No. 24-03084-01 As a result of the inspection conducted on May 6 and 7, 1982, and in accord-ance with the NRC Enforcement Policy, 47 FR 9987 (March 9,1982), the follow-ing violation was identified:
License Condition No. 14.A. states that teletherapy sources shall be tested for leakage at intervals not to exceed six months. Records of test results shall be kept in units of microcuries and maintained for inspection by the Commission.
Contrary to this requirement, it was learned from statements of licensee representatives that leak tests of the cesium-137 source were not performed every six months. Records of the leak test results of the cesium-137 and cobalt-60 teletherapy sources were not maintained for inspection.
This is a Severity Level V violation (Supplement VI).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Consideration may be given to extending your response time for good cause shown.
