5.0 TITLE

Licensed Operator Requalification

5.1 INTRODUCTION

5.1.1 Purpose .

This procedure provides instructions for the conduct I and documentation of the Licensed.0perator Requalifi- {'

cation Program. ,

5.1.2 Applicability .

This procedure applies to all NRC-licensed Operators  ;

and Senior Operators in the Plant Organization,  !*

. including licensed staff members.

5.2 PROGRAM OBJECTIVE The objective of the Licensed Operator Requalification Program is to assure that licensed operating personnel remain competent to operate the plant in a safe, reli.able and efficient manner, par-ticularly under emergency or abnormal conditions.

5.4 RESP 0f4SIBILITIES in 5.4.1 Training Coordinator The Training Coordinator provides supervision for all phases of licensed operator requalification and is responsible to the Resident Manager for the performance and documentation of training in accordance with this procedure and References 5.3.1 through 5.3.6.

5.4.2 Others The Resident Manager, the Superintendent of Power and the Operations Superintendent have tie specific responsi-bilities assigned to them in Section 5.5, Procedure.

5.5 PROCEDURE 5.5.1 General

a. The programs and administrative controls established by this procedure shall be enforced in continuous manner throughout the life of the Plant.
b. Those licensed staff members who are actively engaged in the implementation of the requalification program and other training programs may not be required to i

complete those sections of the program for which they serve an active function. For example, a licensed staff member who prepares and grades a quiz may not be required to take that quiz. This exclusion shall be limited to one licensed individual.

The Licensed Operator Requalification Program consists of

.the following:

a. Regularly-scheduled lectures
b. Self-study reading assignments
c. On-the-job training, including reactivity control manipulations
d. Annual written examinations -
e. Observation and evaluation, including simulation of emergency and abnormal conditions
f. Periodic retraining utilizing a control room simulator.

5.5.3 Schedule j

a. The requalification program shall not exceed periods of two years in length. Successive requalification  ;

programs using the same format shall follow the first i

in a continuous cycle not exceeding two years in length,
b. Replacement operators and senior operators who are granted licenses by the USNRC within 90 days of a scheduled annual written examination may be exempted from that examination. They shall, however, partici-pate in the requalification program throughout the -

subsequent year to the extent required by this proce-dure and their performance on the USNRC written exam-ination(s),

c. Schedules for technical training shall be developed annually by the Training Coordinator.

5.5.4 Technical Training

a. Lectures - The requalification program shall include preplanned lectures to be given throughout the requal-ification period. Lectures are normally scheduled to

5.5.5 Individual Study - Reading assignments shall periodically be made for licensed personnel. The subjects covered shall include but need not necessarily be limited to the following areas:

a. Facility design changes which significantly alter operating characteristics.
b. Procedure changes which significantly affect procedural intent.
c. Facility license changes.

d. Safety-related incidents, including siginficant personal errors, which have occurred at this facility or similar facilities.

Any of the above items may be presented through formal classroom training in lieu of reading assignments.

5.5.6 Special Procedures Review -

~

Each licensed operator and each licensed senior operator shall review the contents of the Special Procedures once each requal-ification year through a combination of the following methods:

a. In-plant simulation of the actions to be taken and the conditions to be verified or observed.
b. Self-study reading assignments. .
c. Formal classroom instruction.
d. Practice at a simulator which reproduces the plant's general operating characteristics, and where the arrangement of controls permits effective use of plant procedures.
e. Implementation of Special Procedures when required by Plant conditions.

The Training Coordinator shall maintain a record of the review of Special Procedures by each licensee.

5.5.7 On-the-Job-Trainina

a. Control Manipulations - General Licensed personnel shall participate as much as possible in control manipulations involving reactivity changes or responses to abnormal conditions to demonstrate their skill and/or familiarity with control systems. Personnel with senior operator licenses are credited with control manipulations if they direct or evaluate the manipulations as they are performed.

Each licensed operator shall manipulate the controls, and each licensed senior operator shall manipulate the controls or direct the activities of licensed operators during plant control manipulations, a minimum of ten (10) times during the term of his license.

In the following list, items designated (*) shall be performed at least annually. Items designated (**)

shall be performed at least biennially (every two years).

  • 1. Reactor startup to the point of adding heat.
  • 2. Reactor plant heatup of 500F or greater.
    • 3. Plant and reactor shutdown (power operation .

to source range). *

  • 5. Power changes of 10% or greater using recircu-lation flow control in manual.
  • 6. Manual control of feedwater during startup or shutdown (reactor critical).
7. Control rod interchange (at power).
8. Shutdown margin testing.
9. Control.rnd scram insertion time testing.
10. Startup or shutdown of a recirculation pump (reactor critical).
11. Adjustments of control rods or recirculation flow to compensate for transient conditions.
12. Turbine stop valve testing.
13. Reactor operations involving emergency or special procedures where reactivity is changing.
14. Changing of pressure regulator setpoint (reactor i critical).

c. Abnormal Control Manipulations As a minimum, abnormal control manipulations must be i walked through with, and evaluated by, a member of the training staff. Any licensed operator or senior 4

I operator may be credited with actions taken during an abnormal event in the plant. In addition, a control room simulator which reproduces the plant's general' operating characteristics may be used to i satisfy the requirements for abnormal control manipulations. However, caution should be exercised to assure that the arrangement of controls on the simulator permits effective use of the plant's special or emergency procedures. During walk-through and simulatior., the use of the plant's Technical Specifi- -

cations should be maximized.

In the following list, items designated (*) shall be performed or walked through at least annually. The balance of the items shall be performed or walked through at least biennially (every two years). *

  • l. Loss-of-Coolant Accident a) Inside and outside primary containment b) Large and small breaks, including leak rate determination.
2. Loss of instrument air,(plant-specific simulator).
3. Loss or degradation of electrical power supplies (plant-specific simulator).
4. Loss of coolant flow.
6. Loss of service water.
7. Loss of shutdown cooling.
8. Loss of component cooling system or cooling to an individual component.
9. Loss or failure of normal feedwater system.
  • 10. Loss of all feedwater (normal and emergency).
11. Loss of protective system channel.
12. Dropped control rod.
13. Inability to move control rods.
14. Conditions requiring the use of standby liquid '

control. .

15. Fuel clad failure or high activity in the reactor coolant or off-gas.

16. Main turbine or generator trip.
17. Malfunctions of automatic control system (s) which affect reactivity.
18. MaTfunction of pressure regulation.
19. Reactor scram.
20. Main steam line break (inside and outside primary containment).
21. Nuclear instrumentation failures.

5.5.8 Maintenance of Staff Licenses Licensed staff members, with the exception of the Superintendent of Power, the Operations Superintendent and the Assistant to the Operations Superintendent, '

shall perform the duties of the Nuclear Control Operator under instruction, or the duties of the Shift Supervisor, either solo or under instruction, for not less than four hours per month when averaged over each calendar year. Licensed staff members shall document operating experience in accordance with Section 5.6 of this procedure.

5.5.9 Inactive License In accordance with 10CFR 55.31, any licensed operator

- or licensed senior operator who, for any reason, has not been actively performing the functions of an oper-ator or senior operator for a period of four months or longer shall, prior to resuming activities for which he is licensed, complete the following require-ments to demonstrate that his knowledge of facility operation and administration are satisfactory:

a. Complete all reading assignments made during the period of his inactivity.
b. Review all changes in operating and administrative procedures, license changes, standing orders, and instructions for the period of his inactivity.
c. Review with the Training Coordinator, or his dele-gated representative, material from all requalifi-cation lectures which were missed.

d. Successfully complete an oral examination and evaluation approved by the Operations Superintendent to demonstrate that his knowledge and understanding of the facility is satisfactory,
e. The facility licensee must certify to the NRC that the operator or senior operator is requalified.

This certification shall be initiated by the.

Resident Manager. .

5.5.10 Written Examinations

a. Written examinations shall be given annually. The examinations shall closely parallel the NRC written examinations outlined in 10CFR 55.21 and 10CFR 55.22.

Annual examinations shall be used to evaluate the '

effectiveness of the requalification program and to identify areas where technical training is needed to upgrade licensed operator and senior operator knowledge.

b. Written examinations shall be graded and maintained in training files,
c. An operator scnring above 80% in all sections of the annual requalification examination may not be required

- to attend further requalification lectures until the next annual requalification examination. Other opera-tors may be excused from lectures in subjects for which they scored above 80% but shall be required to attend lectures on all other topics.

d. An over-all grade of 80*; and 70% in each category shall be considered passing. Should an individual fail to obtain these results, he shall be given an oral examin-ation within two weeks by the Operations Superintendent and the Training Coordinator to determine his competency with respect to performance of licensed activities.

Regardless of their decision, he shall be given addi-tional training in the indicated areas of weakness and a follow-up examination shall be administered within five weeks of his failure to pass the annual examination.

e. Operators who score less than satisfactory on the follow-up examination shall be placed in an accelerated requalification program consisting of '

lectures and " walk-through" sessions. Individual programs shall be tailored. to place emphasis where required and may consist of any or all of the following:

a. Individual reading assignments
b. Additional on-shift evolutions
c. Individual review with members of the operating staff
d. Attendance at formal lectures
e. Simulator training The exact scope and duration of individual acceler- '

ated requalification programs shall be determined by a review team consisting of the Superintendent of Power, the Operations Superintendent and the Training Coordinator. Individuals participating in accelerated requalification shall be relieved from duties requiring a license.

f. Written quizzes shall be administered following required attendance at a requalification lecture or series of lectures on a given topic. In addition, written quizzes may periodically be administered to determine the operators' knowledge of material covered in lectures, specific reading -

assignments or walk-through exercises. These quizzes shall be graded and their results placed in individual training files. A passing grade for these quizzes is 80*.. An operator or senior operator scoring below 80" shall be required to repeat the specific lecture or reading assignment. covered by the quiz.

a. At least annually, the Training Coordina'ar shall schedule " walk-through" interviews for each " licensed >

operator and senior operator. .

b. Supervisors performing " walk-through" interviews will forward to the Training Coordinator a report of the results of each interview. This report shall contain a list of the procedures covered, and the Supervisor's t

evaluation of the licensed operator's knowledge of , the contents of the procedures and ability to take the required action.

c. " Walk-through" interviews shall include the simulation of emergency and abnormal conditions, discussions of required actions to be taken and the reasons for them, responses to alarms and instrument signals, and the operation of selected auxiliary equipment important to plant safety.

5.5.12 Qualifications of Instructors Training center and facility instructors who teach simulator courses or provide instruction in systems, integrated responses and transients shall demonstrate their competence by successful completion of a UStiRC senior operator license examination, except that, at the discretion of the Training , Coordinator, non-licensed staff members and others may provide instruction in areas of their particular expertise. Other, forms may be used wfiich fulfill the intentions of the included forms. 5.6.1 Files

a. Individual Training Files The Training Coordinator is required to maintain a training file for each member of the Plant staff organization. A section of this file, entitled
                      " Licensed Operator Requalification", shall be used to document individual participation in this program.                    ,                 ,
b. Requalification program File '

Records pertaining to a requalification program cycle shall be maintained in a central file by the Training Coordinator. An " active" file shall contain records for the current (incomplete) requalification cycle.

                      " Inactive" files shall contain records for completed requalification cycles.     " Inactive" Requalification Program Files may be placed on microfiche or other suitable data storage medium, but shall be retained

. for the life of the facility. These files shall contain:

1. Copies of annual schedules, as provided for in Section 5.5.3.
2. Reports of formal technical training performed and associated attendance data.
3. Copies of written and oral examinations administered and the responses provided by the examinees. -
4. Completed reading assignments and simulator and in-plant operating experience data.

a. Trainirig Report (Figure 5.1)

At the completion of formal technical training, instructors shall forward this completed form to the Training Coordinator or his representative. Appropriate examinations or quizzes shall be attached. Data shall be transcribed onto indi-

  • vidual requalification program training histories.
b. Formal Trainin'g History (Figure 5.2)

This form shall be used to document individual ' participation in formal technical training, simulator training, and written and oral examin-ations.

c. Simulator Training Attachment (Figure 5.3) r This form, or other suitable records provided by the simulator facility, shall be used to document individual participation in simulator requalifica-tion training. It shall be completed for each participant and attached to a Training Report.
d. Report of Oral Interview (Figure 5.4)

This report shall accompany the oral interview plan prepared by the Training Coordinator.

e. Licensed Operator Self-Study Assignment (Figure 5.5)
