BELLEFONTE NUCLEAR PLANT UNITS 1 AND 2 - LAMBDA POWER MODULE FAILURES -
BLRD-50-438/81-05, BLRD-50-439/81 SECOND REVISED FINAL REPORT The subject deficiency was initially reported to NRC-0IE Inspector R. W. Wright on December 18, 1980 in accordance with 10 CFR 50.55(e) as NCR BLN EEB 8006. This was followed by our first interim report dated January 19, 1981, our final report dated July 21, 1981, and our revised final report dated March 23, 1982. As discussed with R. V. Crlenjak by telephone on June 4, 1982, enclosed is our second revised final report.
We consider 10 CFR Part 21 applicable to this deficiency.
The reason for the revision is to change the expected completion date of the corrective action. The date was not met because additional discrep-ancies have been identified and documented which prevent the completion of the necessary work by the previously scheduled date.

SECOND REVISED FINAL REPORT Description of Deficiency In the procecn of cleaning and preparing capacitor charge plates for Lambda type LX3-E-24-R power supplies, a solvent was applied which advercely affected the dielectric cubstance. The colvent's ccntaminaticn of the dielectric recu,lts in an inability to fully charge the crpacitor plates and r.ay result in effectively chort circuiting the capncitor.
Safety Implications The chart circuiting of the subject capacitors could result in the power supply's diode failure and cubsequently in a discontinuation of a re;:ulated power cource. Loca of power supply in the eccential controls and instrumentaticn uould degrade the safe operaticn of the plant.
Corrective Action The Bailey Meter Company cupplies the subject printed circuit boards (PCB) for the La:.bda type LX3-E-24-R pcwer supplies. Bailey Meter has inforced the I:S23 cupplier (Babcock & Uilcox) that they will change their procedure concerning cleaning of the PC3's.
In the future, Bailey Meter Cc pany will clean the subject PCB's without the subject capacitora installed. A halogen cleaning colvent will continue to be used.
Bailey Meter Ccmpany hac informed the capacitor manufacturer, Sprague, that halogen co1 vent ic reacting with the exposed edge foil of their capacitorn. Sprague, in turn, has remedied the process by epoiy ccating the exposed foil.
TVA will continue to monitor thec" power cuppliec in crder to ensure that the implonantaticn of the new Bailey Meter procedures is effective.
