APPENDIX A NOTICE OF VIOLATION
'"? 1 6 1375 Long Island Lighting Company Docket No. 50-322 This refers to the inspection conducted by representatives of the Region I (Philadelphia) office at the Shoreham Nuclear Power Station, Shoreham, New York, of activities authorized by NRC License No. CPPR-95.
During this inspection conducted on April 0-12, 1979, the following apparent items of noncompliance were identified:
A.
10 CFR 50, Appendix B, Criterion V, states, in part, that:
"Activi-ties affecting quality shall be prescribed by documented instruc-tions, procedures,... and shall be accomplished in accordance with these instructions, procedures...."
The Shoreham FSAR, Chapter 17.1.5A, states, in part, that:
"The LILC0 EQA program establishes provisions for assuring that acti-vities affecting quality of safety related structures, systems, and components during the construction phase of the nuclear power station are performed.... in accordance with approved instruc-tions, procedures, and drawings."
General Electric Specification 22A-2724, Revision 1, states, in part, in' paragraphs:
"4.1.2.4 In-Plant Storage... In-plant storage shall meet the minimum envircnmental requirements specified in table I for up to 1 year storage..." Table I invokes the require-ments of paragraph 4.1.2.1.2 " Internal cleanliness of equip-ment... shall be maintained by closing openings where possible by plugging, capping, and/or sealing with tape...."
Contrary to the above, on April 9, 1979, the electrical junction box idB-580, seven pipe joints, the turbine and pump couplings, and the pump bearing housing of the high pressure coolant injec-tion pump E41-TU-002 were open and exposed to construction dirt and water.
In addition, the residual heat removal pumps, Nos.
Ell-P014A, B, and D, and core spray pump, E21-P013B, mechanical seal piping were unsealed.
This item is an infraction.

The Shoreham FSAR, Chapter 17.1.5A, states, in part, that:

The LILCO Quality Assurance Manual,'Section 16, Corrective Actions, states, in part, that:
" Procedures shall be utilized to assure that conditions adverse to quality... are promptly identified and appropriate corrective action taken."
The LILC0 Startup Manual, Section 4.6, Deficiencies, states, in part, that:
"During the testing program it is necessary to assure that conditions ad. verse to quality or operations... are promptly identified and corrected."
Contrary to the above, on April 9,1979, the nonconforming storage conditions associated with the -high pressure coolant injection pump, E41-TU-002 were identified and reported to the responsible LILCO operations and quality assurance personnel.
No objective evidence of any corrective actions were available on April 12, 1979, to indicate that prompt corrective actions were initiated.
This item is a deficiency.
C.
10 CFR 50, Appendix B, Criterion IX, states, in part, that:
. welding (is)... accomplished... in accordance with appli-cable codes, standards, specifications, criteria, and other special requirements."
l The Shoreham FSAR, Chapter 17.1.9A, Control of Special Processes, l
states, in part, that:
"The program requires that organizations performing special processes on safety related equipment... do so to approved procedures (and)... that all special processes be accomplished... in accordance with applicable codes, standards, specifications and other special requirements."

Appendix A 3
The " Component Checklist" for pipe restraint 1E21-PSR-040 specifies Fitup Sketch NW-1000, Revision 1, " Types of Attachment Welds for Classes 1, 2, and 3 Components." NW-1000 requires full penetration welds.
Contrary to the above, on April ll,1979, pipe restraint lE21-PSR-040 exhibited lack of penetration on one of the integral lugs depicted on drawing No. M-12125-27-3, sheet 2 of 3.
