APPENDIX A NOTICE OF VIOLATION Memphis State University Docket No. 50-538 AGN-201 Training Reactor License No. R-127 As 3 result of the inspection conducted on October 19-21, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66764 (October 7,1980), the following violations were identified.
A.
Technical Specification 6.4.1 requires that the Reactor Safety Committee (RSC) shall meet at least once each calendar quarter.
Contrary to the above, RSC meetings were not held each calendar quarter in that there was no meeting during the four intervals of December 19, 1978 through May 9, 1979, October 23, 1979 through July 15, 1980, July 15, 1980 through January 29, 1981 and January 29, 1981 through October 21, 1981.
This is a Severity Level V Violation (Supplement I.E.).
B.
Technical Specification 6.4.4 requires the Reactor Safety Committee to audit five specific areas of reactor operation at prescribed frequencies.
Contrary to the above, audits were not conducted at the prescribed frequencies in that, annual audits were not conducted during 1977, 1978, or 1980 in the areas of Technical Specificat;on Performance nor in the area of Staff Performance, Training and Qualifica. ions.
Additionally, RSC audits have never been performed in the two area. of Emergency Plan and Procedures or Corrective Actions.
This is a Severity Level V Violation (Supplement I.E.).
C.
Technical Specification 6.4.3 requires the Reactor Safety Committee to review events which have bee i reported in writing within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> to the NRC.
Contrary to the above, the R;C has not reviewed all events reported within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> to the NRC in thats three reported events occurring on March 5, 1979, November 16, 1979 and June 8,1981 have not been reviewed by the RSC.
This is a Severity Level V Violation (Supplement I.E.).
D.
Technical Specification 4.2.d requires a monthly channel test of the shield water temperature, shield water level, and seismic displacement safety channel s.
Contrary to the above, during the interval October 13, 1980 through December 5,1980, a channel test of these three safety channels was not 820423CVN

This is a Severity Level VI Violation (Supplement I.F.).
Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged viola-tions; (2) the. reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
The responses directed by this Notice are not subject to the clearance procedures of the Office of lianagement and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
