Confirmation of Action - Brunswick 1 and 2 Docket Nos. 50-324 and 50-325 This refers to the telephone conversation held on April 8,1982, between you and R. C. Lewis of this office concerning recent events involving misalignment of three components in safety-related systems. The misaligned components were a residual heat removal service water system circuit breaker; a high pressure coolant inje.Gion system steam pressure sensing line isolation valve; and a cooling water outlet valve on a diesel-generator coolant system.
With regard to the matters discussed, it is our understanding that you have or will initiate the following actions:
1.
Conduct an investigation of the events of: Itarch 25, 1982, involving an open residual heat removal service water system breaker; fiarch 21, 1982, involving a closed isolation valve on the steam pressure sensing line to the high pressure coolant injection system; and flarch 21, 1982, involving a closed cooling water outlet valve in the diesel generator coolant system.
The investigation team will include representatives from the CP&L Corporate quality assurance and training staffs. As a minimum, your investigation will determine:
a.
The cause of the misaligned valves and breaker. This includes, but is not limited to, a review of checklists, procedures, training, shift turnover, and control panel indications, b.
The reason for the misaligned valves and breaker not being subjected to independent verifications as required by NRC Order of July 10, 1981.

The need for an in-depth evaluation of maintenance and repair activities which may affect the availability of safety-related systems or contribute to their misalignment.
2.
We understand that the Corporate Nuclear Safety Review Staff has already completed an in-depth review of the misaligned valves and circuit breaker.
l Upon your completion of the total investigation, as discussed above, and the determination of your planned corrective actions, a meeting will be scheduled at our mutual convenience at the Region II office to discuss this ma tter.
This meeting is to be held on or about June 1,1982.
