Subject: Report No. 50-302/82-03

This refers to the special safety inspection conducted by Mr. N. Economos of

this office on February 2-18, 1982, of activities authorized by NRC Operating

License No. DPR-72 for the Crystal River facility.

Our preliminary findings

were discussed with Mr. T. C. Lutkehaus, Nuclear Plant Manager, at the conclu-

sion of the inspection.

Areas examined during this inspection included makeup line thermal sleeve pipe

crack. Within these areas, the inspection consisted of selective examinations of

procedures and representative records, interviews with personnel, and observa-

-

tions by the inspector.

During the inspection, it was found that certain activities under your license

appear to violate NRC requirements.

This item and references to pertinent-

requirements are listed in the Notice of Violation enclosed herewith as

Appendix A.

Elements to be included in your response are delineated in

Appendix A.

Two new unresolved items are identified in the enclosed inspection report. These

items will be examined during subsequent inspections.

In accordance with 10 CFR 2.790(a), a copy of this-letter and the enclosures will

be placed in the NRC's Public Document Room unless you notify this office, by

telephone, within ten days of the date of this letter and submit written appl 1-

cation to withhold information contained therein within thirty days of the date

of this letter.

Such application must be consistent with the requirements of

2.790(b)(1).

The responses directed by this letter and the enclosures are not subject-to the

clearance procedures of the Office of Management and Budget as required by the

Paperwork Reduction Act of 1980, PL 96-511.
