Crystal River Unit 3 Docket No. 50-302 Operating License No. DPR-72 IE Inspection Report 82-02

Dear Mr. O'Reilly:

We offer the following response to the violation listed in the subject inspection report.

NOTICE OF VIOLATION Technical Specification 6.8.1 requires adherence to procedures covering applicable activities recommended in Appendix "A" of Regulatory Guide 1.33, November,1972.

Regulatory Guide 1.33, Section 1.5 requires procedures for the control of maintenance which includes a method of obtaining permission for work to be accomplished and a method for logging such work.

Compliance Procedure CP-ll3, Procedure for Handling and Controlling Work Requests, Revision 29, Section 5, requires Parts I, II, and the Nuclear Shift Supervisor's work authorization signature of Part III to be completed prior to commencing work.

Contrary to the above, review of records on February 1,1982, indicates that work conducted during the period of November 16-26, 1981 in accordance with work request #24403 and modification 81-5-20, was conducted without prior Nuclear Shift Supervisor authorization.

Also, contrary to the above, on February 4, 1982, maintenance was conducted on the Reactor Building pressure transmitter (BS-16-PT) prior to Part II and the Nuclear Shif t Supervisor's authorization in Part III being completed.

This is a Severity Level IV Violation (Supplement D.3.).

Florida Power Corporation concurs with the stated violation, and offers this response:

- Responsible personnel have been individually counseled as to seriousness of this violation, and a letter has been forwarded to all concerned personnel re-emphasizing the importance of adhering to procedures, and notifying disciplinary action will be taken should a like event recur.

Florida Power Corporation is concerned over the management control systems that permitted this violation to occur, and the following additional actions have been taken to improve the effectiveness of our management control systems:

A computerized control of maintenance planning by work request number has been instituted, and a daily printout of all planned maintenance is given to the Shift Supervisor on duty.

- The Operation's Planning Coordinator has been given the responsibility of ensuring the NSSOD is notified of any changes or additions to the printout.

We feel that this daily update of maintenance already in progress, and maintenance to be performed, will enhance our management controls systems to ensure that violations of this type will not recur.
