APPENDIX A NOTICE OF VIOLATION Power Authority of the State of New York Docket No. 50-333 James A. FitzPatrick Nuclear Power Plant License No. DPR-59 As a result of the inspection conducted on April 1-30, 1982, and in accordance with the NRC Enforcement policy (10 CFR 2, Appendix C) published in the Federal Register Notice (47 FR 9987) dated March 9,1982, the following violation was identified:

10 CFR 50, Appendix B, Criterion V requires that activities affectirg quality be prescribed by documented instructions, procedures or drawings.
The James A. FitzPatrick Nuclear Power Plant Standing Order No. 5, " Control of Drawing and Technical Manuals," Revision 0, Section 7.1.1 requires that the master drawing file be maintained to reflect the "as-built" condition of the plant.
Contrary to the above, as of April 30, 1982, the licensee failed to accurately depict the as-built condition of the High Pressure Coolant Infection System (HPCI) on drawing FM-25 and failed to provide a ccmplete valve checkoff list in the HPCI operating procedure in that a total of nine vent, drain or Integrated Leak Rate Test Connection valves were not shown on the drawing or included in the valve checkoff list.
This is a Severity Level IV Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, The Power Authority of the State of New York is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, includina:
(1) the corrective steps which have been taken and the results achieved; (2) corrective step:: which will be taken to avoid further violations, and (3) the date when full ccmpliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.
