Phoenix, Arizona 85009 License No. 02-19728-91 As a result of the inspection conducted on April 6, 1982, and in accordance with the MRC Enforcement Policy (10 CFR Part 2, Appendix C), 47 FR 9987

-(March 9, 1982), the followigg violations were identified:

10 CFR 20.205(c)(1) states that each licensee, upon receipt of a package of radioactive material in excess of the Type A quantities, shall

monitor'the radiation levels external to the package.

4 Contrarv to the above requirement, at the time of the inspection, radioactive packages of cobalt-60 and iridium-192 containing activities of up to 120 curies, which were received on five different occasions in Union, New Jersey between August, 1981 and March, 1982, were not monitored to determine external radiation exposure levels.

10 CFR 34.26 states that each licensee shall conduct a quarterly physical inventory to account for all sealed sources received and possessed.

10 CFR 34.26 also states that the records of the inventories shall be _ maintained.for inspection by the Commission, and shall include the-quantities and kinds of byproduct material, location of sealed sources, and the date of the inventory.

C'ontrary to the above requirements, quarterly physical inventories had not been conducted for.one cobalt-60 and four iridium-192 sources possessed in Union, New Jersey, during the period between the date of license issuance, July 20, 1981 and the date of the inspection, Anril 6, 1982.

This is a Severity Level V Violation (Supplement VI).

-10 CFR 34.28(b) requires the licensee to keep records of inspection 2

and maintenance of radiographic exposure devices, storage containers,.

and source changers which are performed at intervals not to exceed three months or prior to the first use thereafter to assure proper functioning of components important to safety.

Contrary to the above requirement, at the time of the inspection, records had not been~ maintained of the inspection and maintenance of two radiographic exposure devices (ganna camera serial' numbers 2194 and 2445) which was performed on or about Noeember 3, 1981, a period in^ excess of 5 months.

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved. Consideration may be given to extending your response time for good cause shown.
