i Appendix NOTICE OF VIOLATION Miller - Dwan Hospital and Medical Center License No. 22-16390-01 License No. 22-16390-02 As a result of the inspection conducted on April 6,1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9, 1982), the following violations were identified:
License No. 22-16390-01 1.
License Condition No. 16 requires that licensed material be possessed and used in accordance with the statements, representations, and procedures contained in application dated May 30, 1980; letters dated September 23, 1980, and March 12, 1981, and ALARA program dated April 20, 1981.
Item No. 1 of the September 23, 1980, letter states that procedures for calibration of the dose calibrator from Appendix D of the Medical Licensing Guide will be followed. Appendix D requires quarterly linearity tests on the dose calibrator.
Contrary to the above requirement, linearity tests on the dose calibrator have not been performed since October 1980.

10 CFR 20.203(c) requires that each high radiation area be conspicuously posted with a sign or signs bearing the radiation caution symbol and the words:
" Caution High Radiation Area".
Contrary to the above, on the day of the inspection, the high radiation area in your cobalt-60 teletherapy room was not posted as required.
l This is a Severity Level V violation (Supplement IV).
I Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective j
action taken and the results achieved; (2) corrective action to be taken to avoid t
further noncompliance; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for e cause shown.
