i APPENDIX _A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-272 Newark, New Jersey 07101 50-311 Salem Nuclear Generating Station, License No. DPR-70 Units 1 and 2 DPR-75 As a result of an inspection conducted at Salem Units 1 & 2 and at the corporate office in Newark on February 8-11 and 17-18, 1982 and in accordance with Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:

10 CFR 50, Appendix B, Criterion XVIII states in part, "A comprehensive system of planned and periodic audits shall be carried out to verify compliance with all aspects of the quality assurance program and to determine the effectiveness of the program."

Public Service Electric and Gas Quality Assurance Instruction 18-1, Attachment 1, Revision 2, paragraph a requires that the summary of results section include "an evaluation statement regarding the effectiveness of the quality assurance program elements which were audited."

Contrary to the above, the following six audit reports completed in 1981 did not contain an evaluation statement regarding the effectiveness of the quality assurance program elements audited:

S-81-5, Audit of Technical Specifications Compliance S-81-6, Audit of Fire Protection Program S-81-16, Audit of Operating Experience Feedback Program S-81-25, Audit of Design Division of the Engineering Department S-81-30, Audit of Master Drawing /PSBP List S-81-35, Audit of DCR Implementation This is a Severity Level VI Violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office, within thirty days of the date of this notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response time.

The responses directed by this notice are not stabject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
