Dear Mr. O' Reilly:
N-On February 16, 1982 your staff was notified that centrifugal charging pumps "A" and "B" were declared inoperable due to cavitation, and gas was suspected of being in the Chemical and Volume Control System suction piping.
Pursuant to T.S.3.1.2.4 which is reportable per T.S.6.9.1.12(e), Reportable Occurrence Report R0-369/82-15 was to have been submitted by February 26, 1982. At this time, investigation of this event is not yet complete. The complete report will be submitted by March 8, 1982.
