,0 Office of Nuclear Reactor Regulation Items of Interest Week Ending July 30, 1993 Palisades To follow-up open items identified by the Palisades AIT, NRR and Region III 1

staff have held daily conference calls with the licensee to discuss the progress of their ongoing root cause analyses, recovery activities, and short term corrective actions. Activities for the last week are summarized as follows:

Fuel Failure Investication The licensee performed eddy current tests on fuel rods in selected assemblies that have been in core corner locations, particularly in location B-19 wnere the failed fuel rod was discovered. Test results showed cladding degradation in some corner rods.

Based on these inspection results, the licensee established specific bundle replacement criteria, which led to their decision to use three-time burned "L" bundles to replace the 16 corner assemblies.

Furthermore, the licensee plans to reconstitute the replacement "L" assemblies by placing solid stainless steel rods at each assembly corner location. The staff is currently reviewing this proposal.

Uoner Guide Structure (UGS) Stuck Assembly Root Causes The reactor lower core support plate holes and the stuck assembly SAN-8 upper tie plate holes were gauged and found acceptable. The UGS alignment pins at two locations were found bent.

Combustion Engineering has been contracted to replace these pins starting this week.

Root cause analyses are continuing.

Recovery of Failed Fuel By measuring activity levels on the CVCS filters and demineralizer, the licensee estimated that about 8 percent of the lost fuel can be accounted for.

Efforts are continuing to locate, quantify, and remove the missing fuel in the vessel.

Fuel Failure Detection Plan The licensee contracted NWT Corporation to review the radiochemistry data from the last cycle. ' NWT will perform a detailed review of Palisades fuel monitoring procedures.

4 Limerick Generatino Station. Units 1 and 2 Both Limerick units are operating at 100% power with no significant problems.

PECo will start the transfer of the Shoreham fuel to the Limerick site at the -

2 transport of the slightly irradiated fuel will leave its storage location in Morris, IL, on July 30, 1993, travel by rail to Eddystone, PA, then by barge to the Shoreham site. The loaded cask is scheduled to leave the Shoreham site on September 23, via barge to Eddystone, PA, and continue by rail to the Limerick site.

It will reach the Limerick site by the end of September. The total number of trips are expected to be 33, and the full transfer is expected to be completed by spring 1994. The staff authorized PECo to receive, possess, and use the Shoreham fuel at the Limerick facility on June 23, 1993.

Comanche Peak Steam Electric Station Unit 2 The licensee successfully completed the 100% load reject test on Comanche Peak Steam Electric Station Unit 2 on July 27, 1993. This test completes the initial startup test program for the plant. Unit 2 is expected to restart on July 30, 1993, and should begin commercial operation during the week of August 2, 1993.

NRC Observation of INPO Accreditation Process Dr. George Usova, Training and Assessment Specialist from the Operator Licensing Branch, attended the INP0 Accreditation Board meeting in Atlanta, Georgia on July 27 and 28, 1993. The purpose of Dr. Usova's attendance was to observe the INP0 Accreditation process. NRC observation is provided for all INPO Accreditation Board meetings on a regularly scheduled basis.

Fire Barrier Testina Planned by Darchem Enaineerina Representatives of Darchem Engineering informally told the staff on July 20, 1993, that they are planning to conduct fire endurance tests on Thermo-Lag fire barrier installations that have been upgraded using Darchem's fire barrier system. They plan to test 3 24-inch x 6-inch cable trays, of which one will be protected by Thermo-Lag only and two will be enhanced by Darchem's product. The Thermo-Lag will be installed by a certified Thermal Scien e, Inc. installer, and none of the Thermo-Lag joints will be pre-buttered. The testing will be performed in the United Kingdom in two to three weeks. Darchem also mentioned that they are considering flammability testing and testing the effects of lower levels of radiant heat (around 800 *F) on the effectiveness of Thermo-Lag versus Darchem's product.

Darchem plans to provide all test reports to the NRC staff at the completion of the testing.

Boron Credit in PWR Spent fuel Pools A meeting was held on July 21, 1993, with the Westinghouse Owners Group (WOG) and NRC staff to discuss the safety implications of allowing credit for dissolved boron in PWR spent fuel pool water under normal conditions.

Although there is typically approximately 2000 parts per million (ppm) boron ir 9 pool at all times, the present staff position allows credit for boron

' der accident conditions. WOG presented background information, including an overview and benefits which would be realized if credit for boron were allowed in the spent fuel pool under normal as well as accident JULY 30, 1993 ENCLOSURE A

3 conditions. These benefits include additional fuel storage due to closer fuel assembly spacing and lower fuel cycle costs due to increased enrichment, and thus fewer fuel assemblies, per cycle. A generic topical report is planned for submittal to the NRC in August 1994 and a lead plant application in the third quarter of 1994.

BWR Water Level Instrumentation The Reactor Systems Branch met with Commonwealth Edison on July 26 to discuss their plans and schedule for implementation of a modification to their level instrumentation as requested by Bulletin 93-03 for the LaSalle station. At this meeting Commonwealth presented data from tests they performed using a periodic backfill system to flush their indication only reference legs with flow from the CRD system. This data showed spiking of the level indication during various control rod manipulations. The licensee believes that this spiking could result in spurious alarms or trips if this flow were to be applied continuously to reference legs that provide trip signals. The licensee stated that they would not be able to install a modification on the schedule requested by BL 93-03, and that the engineering necessary to complete the design and to resolve their concerns with CRD flow perturbations would not be completed until December of 1993.

The staff has been informed that the Pilgrim plant modified their water level instrumentation this weekend by installing a continuous reference leg backfill system.

The plant is starting up this week. The Reactor Systems Branch will be on site this week to perform a review of the licensees 50.59 evaluation for this modification and to review the licensees post modification testing, with particular attention to the potential for perturbations in the CRD flow to result in spurious trips or alarms, as postulated by Commonwealth Edison.

NRC and FEMA Monitorina Imoact of Midwest Floodina on Offsite Emeroency Preparedness PEPB is consulting daily with FEMA Headquarters counterparts regarding the impact of midwest flooding on offsite emergency preparedness for licensed nuclear power plants in the heaviest flood areas, including Quad Cities (which delayed restart), Cooper (which shut down), Callaway, and Fort Calhoun.

FEMA is currently reviewing offsite emergency capabilities of Nebraska and Missouri, which have been reportedly affected by the flooding.

Maintenance Good Practices Workino Grouc On July 22-23, 1993, Thomas Foley of the Division of Reactor Inspection and Licensee Performance attended the IEEE Nuclear Power Engineering Committee Summer meeting in Vancouver British Columbia, Canada. Mr. Foley is the NRC representative on the Maintenance Good Practices Working Group 3.3 which is developing a manual on maintenance good practices for nuclear power plant electrical equipment. The manual is expected to be completed in approximately 36 months.

Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending July 30, 1993 Counterpart Teleconference on Fuel Cvele Facility Issues On Monday, July 26, 1993, staff from Regions II, III and V and the Division of fuel Cycle Safety and Safeguards participated in a counterpart teleconference to discuss safety issues of common interest involving fuel cycle facilities.

Topics of discussion included: assessment of the safety significance of events and establishing appropriate severity levels; implementation of the revised Part 20, with particular attention to as low as reasonably achievable (ALARA) determinations; issuance of the supplement to Bulletin 91-01 on the reporting of criticality safety e' vents; status of the development of the Standard Review Plan for fuel cycle license applications, renewals and amendments; status of the recently revised enforcement policy; and admini-strative procedures for activation of the Safety Assessment Team. A decision was made to schedule monthly counterpart calls.

Safeauards Summary Event List. NUREG-0525. Volume 2. Revision 1 The Safeguards Summary Event List (SSEL), NUREG-0525, Volume 2, Revision 1, was published in July. This report provides brief summaries of several hundred safeguards-related events involving nuclear material or facilities which occurred during the period January 1, 1990, through December 31, 1992.

URF0 Transition Oversicht Team Meetina The second meeting of the Uranium Recovery Field Office (URFO) Transition Oversight Team (TOT) was held on July 27, 1993. The TOT reviewed and recommended approval of a charter for the team which included items contained in the Staff Requirements Memo for SECY-93-150 " Plan of Action for Carrying Out the Closure of the Uranium Recovery Field Office". The team also finalized an Action Item List which included the initial items identified for oversight related to the URF0 closure.

Issues discussed at the meeting and listed on the Action Item List included plans for a team meeting with the URF0 Staff on September 9,1993, followed by a meeting with licensees and State officials on the same day. The next meeting of the URF0 TOT will be at the URF0 Denver office on September 8, 1993.

Nuclear Reaulatory Commission. Environmental Protection Aaency Manacement Briefed on Risk Harmonization Effort On Wednesday, July 28, 1993, staff from the Nuclear Regulatory Commission and the Environmental Protection Agency (EPA) briefed the Director, Office of Nuclear Material Safety and Safeguards, NRC, and the Director, Office of Radiation Programs and Indoor Air, EPA, on the status of risk harmonization efforts. The March 1992 Memorandum of Understanding between NRC and EPA directed both staffs to explore ways to harmonize risk goals, and to cooperate in developing approaches to risk assessment. This briefing summarized the results of a review of risk management approaches and risk goals in both agencies. By comparing a range of programs that regulate radionuclides [and, for EPA, chemical hazardous substances], staff identified similarities and JULY 30, 1993 ENCLOSURE B

differences in the approaches used by the two agencies. Despite different approaches and regulatory responsibilities, NRC and EPA found that they often achieved similar results in terms of protection of the public.

It was noted that perceptions of differences in protection provided by the two agencies often ignore the significant effect of implementation policies used by NRC and EPA. However, the study also noted that in certain program areas, the two agencies do not agree on what constitutes adequate protection.

Activities of the National Academy of Sciences Committee on the Technical Bases for Yucca Mountain Standards As a follow-up to the Committee's discussion during its May 27-29, 1993, meeting and as requested by the staff officers of the National Academy of Sciences (NAS), tne Nuclear Regulatory Commission staff is continuing to provide assistance on issues that the Committee may address as part of its study. The second meeting of the NAS Committee on Technical Bases for Yucca Mountain Standards has been scheduled for August 26-27, 1993 in Las Vegas, Nevada. The August meeting will focus on scientific and technical bases related to the following topics: the alternative scientific approaches for expressing health-based standards, connections between releases and exposures, and the comparison of health-based and technology-based standards.

Specific issues to be covered include:

time frames of concern, assessing risks to populations and individuals, dose-rencnse relationships, accounting for different radionuclides and different pathways to exposures, spatial and temporal gradients in exposures, and consistency with other U.S. and international standards. The Committee intends to focus on scientific discussions rather than policy positions of individuals or organizations.

Audit of the Lawrence Livermore National Laboratory (LLNL)

During July 19-23, 1993, quality assurance (QA) and technical staff of the Division of High-Level Waste Management (HLWM) observed a QA audit of LLNL performed at Livermore, California, by the Yucca Mountain Quality Assurance Division (YMQAD) of the Department of Energy Office of Civilian Radioactive Waste Management (OCRWM). This audit, YMP-93-14, evaluated the LLNL QA program to determine whether it meets the OCRWM requirements and commitments.

Six QA programmatic elements and four technical areas were evaluated by the YMQAD audit team.

The HLWM observers determined that the audit was effective and agreed with the audit team that the LLNL QA program was effective overall in each of the QA programmatic and technical areas audited. As a result of the audit, one preliminary Corrective Action Request (CAR) was issued by the audit team.

The adverse condition identified in the CAR is not significant in terms of the overall LLNL QA program. The item will be tracked by OCRWM to ensure effective corrective action.

Resolution of Generic Safety ' t,ue B-56. " Diesel Generator Reliability" The NRC has withdrawn a rule amendment to 10 CFR 50.63 proposed in April 1992 vhich would have required licensees to test ar.d monitor emergency diesel generators against specified criteria (58FR37884). In addition, Revision 3 to Regulatory Guide 1.9, " Selection, Design, Qualification and Testing of Emergency Diesel Generator Units Used as Class IE Onsite Electric Power Systems at Nuclear Power Plants," July 1993 was issued. These two actions constitute resolution of Generic Safety Issue B-56, " Diesel Generator Feliability".

This revision to Regulatory Guide 1.9 integrates into a singular regulatory guide pertinent guidance previously addressed in f.evision 2 of Regulatory Guide 1.9, Revision 1 of Regulatory Guide 1.108, and Generic Letter 84-15.

The guidance for the monitoring and maintaining of emergency diesel performance against values selected by licensees in their station blackout coping analyses has un incorporated into Regulatory Guide 1.160, " Monitoring the Effectiveness ci hintenance at Nuclear Power Plants," June 1993.

GEIS Sconina Meetinas On July 26, 27, and 28 staff from RES, NMSS, and OGC conducted public scoping meetings on the Generic Environmental Impact Statement for the enhanced participatory rulemaking on radiological criter4 for decommissioning.

Meetings were held in ti:e afternoon and evening in-San Francisco, California, on July 26; Oklahoma City, OK. on July 27, and Cleveland, OH. on July 28.

Each meeting averaged approximately 30 individuals in attendance, with a total of 52 speakers making statements and providing comments.

In general, all of the speakers supported NRC rulemaking action, and supported either a return to background or restricted use alternative for rulemaking. A number of speakers had questions about the use of the term " generic" and cautioned against allowing industry to leave sites under less restrictive requirements than return to background. Members of the press covered each of the meetings. A number of commentors suggested additional alternatives and impacts to be considered within the scope of the GEIS, including the alternative of a process based rule which would outline the manner in which site specific criteria would be develop using local citizen involvement.

The staff will analyza the comments submitted in the meetings along with written comments submixted via mail on NRC's computer bulletin board for the rulemaking.

An Inteoral SBWR Test Facility Purdue University was selected as the contractor to design, build, and operate an NRC-sponsored integral SBWR test facility. The three-year co. tract was approved and started on July 26, 1993. The integral SBWR txt (acility at Purdue will be a low-pressure, quarter-height scaled model of the SBWR with a volume scale of 1/400.

It will have all of the key components and systems required for investigating the integral performance of the GDCS (Gravity-Driven Cooling System) and PCCS (Passive Containment Cooling System), which are the two passive, safety systems unique to the SBWR. Test results from this integral facility will be used not only to confirm GE's data from the GIST and GIRAFFE test facilities, but also to broaden the data base for assessing SBWR codes for accident analyses.

Earthauake Research Conference A meeting to review research performed by the U.S. Geological Survey during the past year with NRC assistance was held at the University of Washington in Seattle on July 20-22.

The meeting was attended by members of the Structural and Seismic Engineering Branch, RES and by members of the Civil Engineering and Geosciences Branch, NRR. Several participants from Canadian universities and government agencies were also present. The discussions focused on the subjects of faulting and ground motion, regional seismicity, and palemeismology. These subjects were discussed in detail for the Pacific Northwest and the New Madrid, Missouri area, with additional emphasis on recent paleoliquefaction studies.

Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending July 30, 1993 Diaanostic Evaluation and incident Investiaation Branch (DEIIB)

The EDO has approved the overall plans for the Quad Cities Nuclear Station Diagnostic Evaluation (DE). The Quad Cities DE Team kickoff meeting was conducted on July 26-28.

Representatives from the Region III Projects and Quad Cities resident staff, NRR Projects and Licensee Performance Evaluation, and AE00 Performance Indicator Section provided their insights and perspectives on the licensee performance at the Quad Cities Station.. Members of DEIIB and AE0D provided the Team orientation and training on the goals, objectives, methods and procedures for the DE. The team is continuing its

review of documentation that has been provided by the licensee and has initiated development of the functional area evaluation plans.

Incident Response Branch (IRB)

On July 24-25, 1993, the NRC Headquarters Operation's Center was staffed to monitor the flooding in the vicinity of the Cooper Nuclear Station. Staffing was supplemented with agency experts in emergency response, plant operetions, hydrology, and risk assessment. The Federal Emergency Management Agency (FEMA) provided communications support at the site by dispatching a mobile satellite van throughout the weekend. Normal monitoring was assumed on July 25 when water levels decreased throughout the area.

On July 28, IRB staff participated in a limited exercise with Comanche Peak i

and Region IV. The exercise provided NRC Headquarters an opportunity to interface with the State of Texas and the Region.

Office of Administration Items of Interest Week Ending July 30, 1993 F Street Closeout The NRC office space at 80 "F" Street, Washington, D.C. has been closei. This office space was previously used by.the Commissioners and other NRC officlals when conducting activities on Capitol Hill. The agency is relinquishing this space as of July 31, 1993. The removed property has been taken to the NRC warehouse and will be used to meet future requirements.

Contract Award A competitive contract was awarded to Jack Faucett Associates, Inc., of Bethesda, Maryland, on July 27, 1993 entitled, "NRC Rules and Regulations."

The contractor will maintain and continuously update the basic NRC Rules and Regulations as a permanent record of the NRC rulemaking activity. The contract amount is $77,200 and the period of performance is August 1, 1993 to July 30, 1994, with four one-year option periods.

Commissioner Curtiss' Pacers I

On July 22, 1993, the Division of Security (SEC) completed the review of 46 boxes of information identified by former Commissioner Curtiss' office for donation to the Hoover Institution on War, Revolution, and Peace at Stanford University. As a result of this review, approximately 144 items were identified which contained sensitive unclassified information.

These items were withdrawn from the collection and will be held in courtesy storage under NRC control.

SEC has been informed that the documents to be donated will now undergo review by OGC and the Division of Freedom of Information and Publications Services.

Office of Information Resources Management Items of Interests Week Ending July 30, 1993 Comnuter Security Activities On the recommendation of the NRC and several other Federal agencies, the National Institute of Standards and Technology has proposed modifying the Data Encryption Standard (DES) to support software implementation. The NRC is one of two Federal Agencies that applied for a waiver of compliance with the old standard so that a software implementation of DES could be used in the agency.

The revised standard is now undergoing final review before it is submitted to the Secretary of Commerce for approval.

Final publication is expected this fall.

Enerav Science and Technoloav Software Center During the third quarter FY 1993, Energy Science and Technology Software Center distributed 50 NRC sponsored software packages. For FY 1993,148 packages have been distributed to date.

Ukraine State Committee for Nuclear Radiation Safety Under the USNRC-USAID Interagency Agreement, Elizabeth Shelburne and David Drapkin will be visiting the Ukrainian State Committee for Nuclear and Radiation Safety (SCNRS) in Kiev on August 2 through 5.

The purpose of the visit is to gather requirements and other information in order to provide the SCNRS with appropriate document and information management system (s) in support of several NRC priority projects under the Lisbon Initiative.

Communications Security (COMSEC)

STU-3 secure telephone units and Motorola MFax5000T secure facsimile equipment will be installed in Regions 11 and IV this week.

This type of equipment has already been installed in Regions I and III.

The equipment will allow i

transmission and receipt of classified documents by telephone. Previously, the only method of passing classified documents to Regions II and IV had been by mail or courier.

ITS Trainino Lab On July 19, 1993, the new course, AUTOS Through Windows, was piloted at the Information Technology Services (ITS) Training Lab. This course was specifically developed to support users in the Office of Nuclear Regulatory Research who will be accessing AUTOS (Agency Upgrade of Technology for Office Systems) through a Windows platform.

It is designed for users who are novice AUTOS LAN (Local Area Network) and/or Windows users. The users will learn how to access and use the resources on the AUT0S LAN in a Windows environment.

Office of Personnel Items of Interest Week Ending July 30, 1993 Child Development Center Initiatives On July 29, the Board of Directors for the Child Development Center conducted a briefing for NRC employees in the Rockville area.

Information related to the size of the Child Development Center, its total capacity, the planned curriculum, estimated tuition costs, the scholarship program, the interior design including equipment, and the layout of the toddler and preschool playgrounds was provided. The Board will conduct a second briefing for employees in the Bethesda area on August 11.

SES Performance Review Board Meetina Attended Representatives of the Office of Personnel met with the Senior Executive members of the 1993 SES Performance Review Board for a preliminary meeting on July 29.

Preparation for the Board's annual meeting at the end of August are on-going.

National Technical Association Conference At; ended Deirdre Spaulding NRR, Garry Harris RII and Tom Yingst OP attended the National Technical Association 65th Annual Conference in New Orleans, LA on July 21 to July 24. Career Opportunities within the NRC were presented during the exhibit portion of the Conference which included workshops covering topics of concern to the membership.

IAG/CODAT Meetina Attended On July 28, Henry Rubin attended the Interagency Advisory Group Committee on Development and Training (IAG/CODAT). Topics included a briefing on distance learning, a briefing on National Performance Review, and a preview of the FY 1994's Women's Executive Leadership Program and the Executive Potential Program for Mid-Level Employees.

Office of Enforcement Items of Interest Week Ending July 30, 1993 Sianificant Enforcement Actions A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $5,000 was issued on July 28, 1993 to Tulsa Gamn Ray, Inc., Tulsa, Oklahoma. The action was based on a Severity Level III problem involving the loss of a radiography camera from a licensee vehicle. The camera was recovered by a member of the public and returned to the licensee within an hour of the incident. No adjustments to the base civil penalty of $5,000 w'ere considered appropriate.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $ 2,000 was issued on July 28, 1993 to the University of Virginia. The action was based on the operation of the UVAR reactor for approximately 5 1/2 hours without a number of the scram safety channels being operable. During the troubleshooting of a problem with the reactor instrumentation, a senior reactor operator switched two mixer-driver modules which, because of the presence of internal electrical jumpers, were not identical. This resulted in the disabling of the scram functions. The two violations that were identified have been classified as a Severity Level II problem. The base civil penalty of $4,000 has been mitigated a total of 50 percent for the licensee's identification of the self-disclosing event and the corrective actions taken.

A Notice of Violation and Proposed Imposition of Civil Penalties in the amount of $100,000 was issued on July 30, 1993, to Commonwealth Edison Company (Quad Cities Station, Unit 2).

The action was based on two violations involving the inoperability of emergency diesel generators wnich have each been classified as Severity Level III violations.

In the first, the shared emergency diesel generator was inoperable under certain conditions, due to a design deficiency.

in some associated logic circuitry.

In the second, the Unit 2 emergency generator was determined to be inoperable because the associated cooling water pump was incapable of performing its function.

In each case, the escalation and mitigation factors were considered.

In the first case, the base civil penalty of $50,000 was escalated a total of 50 percent to $75,000 after t

partially offsetting escalation for prior opportunities to identify with mitigation for licensee identification.

In the second case, the base civil penalty was mitigated 50 percent to $25,000 based upon mitigation for prior good licensee performance.

Civil Penalties Paid Nebraska Public Power District (Cooper) paid the civil penalties in the amount of $200,000. The action was based on two Severity Level III violations. The first involved providing inaccurate information in response to an NRC Notice 4

of Violation.

The second involved the failure to identify and correct a potentially significant condition adverse to quality involving startup strainers remaining in safety-related systems. The licensee responded to the Notice in a letter dated April 29, 1993. After careful consideration of the JULY 30, 1993 ENCLOSURE K

9 licensee's response, the staff has concluded that the violations did occur as set forth in the Notice of Violation and Proposed Imposition of Civil Penalties and that the escalation and mitigation factors were correctly applied for both civil penalties.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $500 was issued on June 24, 1993, to Hazelton Wisconsin, Inc., Madison, Wisconsin. Hazelton Wisconsin is authorized to possess and-use curie quantities of any byproduct material with atomic numbers 1-83 for use in research and development as defined by 10 CFR 30.4(q) and animal research.

In addition, the company is authorized to possess up to 1.2 curies of Ni-63 in gas chromatographs, each source not to exceed 15 millicuries.

In January 1992 the company accidently discarded one of these sources in the local landfill.

Because of ineffective corrective actions, it accidently discarded a second source in Seotember 1992. These losses constitute a violation of 10 CFR 20.301. The base civil penalty is $500. The base penalty was mitigated 50%

for identification and 100% for licensee performance, but it was also escalated 50% for ineffective corrective actions and for the two occurrences, resulting in no change to the base.

At the July 28 Progress Meeting, the developer reported that the County had issued construction permits for floors 4, 5 and 6 and that floors 2 and 3 and the auditorium will be reviewed under the " fast-track" system but may take longer than the 5-7 day period that is typical under the fast-track review.

On July 29, Office of Consolidation staff and the' Day Care Committee presented general information about the proposed day care center to interested NRC staff.

Office of State Programs Items of Interest Week Ending July 30, 1993 NARUC Summer Meetinas Spiros Droggitis, State Programs, attended the National Association of Regulatory Utility Commissioners (NARUC) Sunner Meetings on July 24-27, 1993 in San Francisco, California. The NARUC Subcommittee on Nuclear Issues / Waste Disposal held a workshop on "The Financial Implications of Spent Fuel Storage Alternatives" on July 24, 1993. Charles Haughney, Chief, Source Containment and Devices Branch, NMSS, made a presentation on NRC's regulations governing interim storage at reactors. The Subcommittee held its meeting on July 25, at which Commissioner Kenneth Rogers made a presentation on the status of NRC issues of interest. Subcommittee Chair Lynn Shishido-Topel indicated that the Subcommittee would be interested in participating in the October workshops with States on issues associated with the environmental review for license renewal. The Subcommittee adopted two resolutions: calling for full funding for research on the Advanced Liquid Metal Reactor and Actinide Recycle System for the next three years and concerning the funding mechanism for the high-level nuclear waste program. These were later adopted by the Committee on Electricity.

Secretary of Energy Hazel O' Leary addressed NARUC on July 27, 1993. Secretary O' Leary indicated her desire for a collaborative resolution to the high-level waste problem.

Industrial Radicaraohv Course State Programs and the Technical Training Center (TTC), sponsored an industrial radiography course on July 26-30, 1993 in Burlington, Massachusetts. The course was conducted by the Amersham Corporation under contract with the TTC. The class consisted of twelve students from State radiation control programs and nine from the NRC.

The course covered various aspects of industrial radiography safety.

Comoatibility Workshop The Office of State Programs conducted a compatibility workshop on July 26-27, 1993 in Rockville, Maryland. The purpose of the workshop was to hold a discussion on the issues associated with the proposed policy on compatibility of Agreement States. Commissioner Forrest Remick provided opening remarks to the workshop participants which included representatives from the NRC, States, industry and environmental groups.

Issues discussed included: What are the core elements of a compatible program?, including uniform national standards, radiation protection standards and interstate commerce considerations; How much flexibility should there be in a compatibility policy?; communications between State and Federal regulators; implementation of the policy, including compatibility determinations and program reviews; and procedures for revocation of an agreement with a State.

Radiation Advisory Committee Meetina Maria Lopez-Otin, Federal Liaison, attended the quarterly meeting of the Environmental Protection Agency's (EPA) Radiation Advisory Committee (RAC) on JULY 30, 1993 ENCLOSURE M

The RAC discussed a number of items of interest to the NRC, e.g. the draft EPA NORM Document, the Cleanup " White Paper" and the RAC initiative on waste classification. Staff may obtain further details from Ms. Lopez-Otin.

LLW and Uranium Recovery Reaulators Workshoo The Office of State Programs conducted a " Low-Level Waste and Uranium Recovery Regulatory Workshop" on July 28-30, 1993 in Rockville, Maryland.

Chairman Ivan Selin provided opening remarks to the workshop participants. The LLW sessions included discussions of performance assessment, engineered barriers, infiltration, source term and groundwater. Also discussed were: revisions to the Standard Review Plan, uniform manifest for LLW, financial assurance, compilation of cost for LLW disposal facilities and LLW storage. The uranium recovery session covered general topics of interest such as: closure of the Uranium Recovery Field Office, termination of Agreement State licenses, Clean Air Act activities and NRC/ State / EPA memorandum of understanding implementation. Other topics included: closure approvals and documentation; license termination; and site transfer and long-term care.

Items of Interest Week Ending July 30, 1993 Royal Thai Senate Committee on Science. Technoloav and Enerav On Monday, July 26, 18 members of the Royal Thai Senate Committee on Science, Technology, and Energy visited the NRC..The Thai Government is considering approving the construction of the country's first nuclear power plant, which might go into operation as soon as 2006. The discussions at NRC, led by Mr.

Carlton Stoiber, Director of the Office of International Programs, and assisted by Mr. Paul Lohaus, Chief, Low-Level Waste Management Branch, NMSS; Mr. Frank Gillespie, Director, Program Management, Policy Development, and Analysis Staff, NRR; and Mr. Martin Malsch, Deputy General Counsel for Licensing and Regulations, 0GC, provided an overview of waste management issues, the creation and implementation of health and safety regulations, and the legal basis of a regulatory program. The group was also scheduled to visit the Limerick nuclear power plant in Pennsylvania, and was going to tour GE's Nuclear Energy Headquarters in Califarnia.

Ukrainian Environmental Minister Visit to NRC On Tuesday, Dr. Yuriy Kostenko, the Ukrainian Environmental Minister visited NRC to meet with Chairman Selin, Commissioners Remick and dePlanque, James Taylor and Carlton Stoiber. Dr. Kostenko presented a proposal for participation by the Environmental Ministry in JCCCNRS programs that dealt with environmental issues. He was very interested in obtaining information, particularly the economic aspects, that would help in developing an overall energy policy for Ukraine. A national energy policy is apparently a prerequisite for lifting the moratorium on start-up of three nearly completed VVER 1000 reactors and closing the two remaining Chernobyl RBMK's.

Responding to Dr. Kostenko's request for U.S. review of the Ukrainian energy policy, Chairman Selin recommended that the request go through the U.S.

Embassy in Kiev and the Embassv would coordinate a composite review from various U.S. sources.

Renewal of NRC-Israel Atomic Enerav Commission Arranaement On Friday, Dr. Gideon Frank, Director General of the Israel Atomic Energy Commission visited the NRC to sign the third renewal of the NRC-IAEC

" Arrangement for the Exchange of Technical Information and Cooperation in Nuclear Safety Matters."

Following the signing, Dr. Frank, accompanied by Dr.

Yona Ettinger, Scientific Attache at the Israeli Embassy in Washington (and himself the former Director General of the IAEC), called on the Chairman and Commissioners Rogers, Remick and de Planque.

I Region I Items of Interest Week Ending July 30. 1993 1.

Enforcement Conferences The Division of Radiation Safety and Safeguards held two enforcement conferences, on July 27, 1993, and on July 29, 1993. The first conference was conducted,with~ Berkshire Medical Systems, Pittsfield, MA, to discuss apparent failures to properly prepare written directives in accordance with the medical Quality Management program, and an apparent lack of management oversight of the radiation safety program.

The second conference was conducted with MPI Pharmacy Services, Inc. (a subsidiary of Amersham Healthcare), Bensalem, PA, to discuss the licensee's apparent deviation from following the information contained as package inserts for processing of radiopharmaceuticals. Both conferences provided NRC staff the opportunity to understand more fully the licensees' actions.

Enforcement action is pending for both cases.

2.

Fit 7 Patrick FitzPatrick declared an Unusual Event at 10:33 a.m. on July 29, 1993, when the licensee found, while performing a biennial procedure review, that APRM surveillance testing did not include testing of the APRM downscale trip function required by Technical Specifications. The licensee declared an Unusual Event as a result of declaring the APRMs inoperable and commenced shutting down the plant from 100% power.

The licensee terminated the Unusual Event at 1:50 p.m. following satisfactory completion of the downscale scram trip for all APRMs and commenced increasing power from 82%.

The licensee concluded that this testing had not been done during the life of the plant.

Region II Items of Interest Week Ending July 30, 1993 1.

On July 28, the Deputy Regional Administrator, accompanied by Regional staff, was at the Oconee facility to present the latest SALP results to representatives of Duke Power Company and to hold a public meet'ng with local officials attending the SALP presentation.

On July 27, 1993, the Foundation for Global Sustainability conducted news conferences at Knoxville and Chattanooga, Tennessee. The conferences were conducted to distribute a report from NUS (a consultant to TVA) that documented a management assessment of quality assurance at Watts Bar. The NUS report had been previously discussed on June 18 in a public meeting held between the NRC and TVA and was under review by the NRC.

Region III Items of Interest Week Ending July 30, 1993 1.

Reaion III Office Soace Vodate GSA is working on the final design intent drawings for the Region III space. We expect to review and sign off on these drawings by August 6, 1993. The office buildout is scheduled for completion 120 days after final approval of the drawings.

If we can keep these activities on schedule the actual move to the new space should take place in early December.

2.

Ball Memorial Hosoital On July 19, 1993, Region III learned that the licensee was investigating unauthorized increases of radiopharmaceutical dosages by two or more technologists at Ball Memorial Hospital in Muncie, Indiana. According to licensee officials dosages were increased from 10-40% of the prescribed dosage in order to decrease the time needed for scanning overweight or very ill patients. The practice had occurred since 1988.

On July 21, 1993, Region III and the Office of Investigations began a special inspection at the licensee's facilities. That effort is continuing.

On July 26, 1993, Region III issued a Confirmatory Action Letter documenting the licensee's agreement to adopt certain corrective actions including double verification of radiopharmaceutical dosage assays.

Zign On July 29, 1993, Region III participated with the Zion Nuclear Power Plant in an Emergency Preparedness Exercise. Although the exercise was originally planned to include full participation with the States of Illinois and Wisconsin, both States withdrew due to resource strains as a result of emergency activities associated with significant flooding on or near the Mississippi River.

Region III limited its play to Incident Response Center activation, real time player selection of both base and site teams, and base team participation with Zion for about two hours.

Perry Nuclear Plant On July 29, 1993, Mr. R. J. Farling,. Chairman, President, and CEO of-Centerior Energy Corporation, and Mr. D. C. Shelton, Executive Vice President - Power Generation of Centerior Energy Corporation, met with the Regional Administrator and other members of Region III management, to directly communicate concerns about adverse performance trends at the Perry Nuclear Power Plant. The meeting was conducted in response to the 1

trends at Perry raised during the June Senior Management Meeting and the need for such a meeting.

Duane Arnold Enerav Center On July 29, 1993, the Regional Administrator and other members of his staff, and Mr. J. N. Hannon, Director, Project Directorate III-3, NRR, and a member of his staff, held an Open Enforcement Conference with Mr.

J. Franz, Jr., Vice President Nuclear, Iowa Electric Light and Power Company, and members of his staff to discuss two apparent violations.

One apparent violation concerned standby diesel generator inoperability and the second concerned crew performance weaknesses. Although this was an "open" enforcement conference, only one member of the public attended (a representative from GE).

Commonwealth Edison Company - IPE On July 27, 1993, the Deputy Regional Administrator and Acting Director for the Division of Reactor Safety hosted a meeting with Commonwealth Edison Company (CECO) to discuss the results of and insights gained from the IPE for Dresden. CECO was led by Dresden's Technical Services Superintendent and Site Engineering Manager.

The licensee discussed their corrective actions related to the environmental qualification of the main feedwater isolation valve bypass valves, a missing T-drain on a residual heat removal system motor operated valve, and a loss of spent fuel pool cooling event. This meeting was open for public observation.
