NOTICE OF VIOLATION Docket Nos. 50-Duquesne Light Company Beaver Valley Power Station License Nos. DPR 66 Units 1 and 2 NPF 73 During an NRC inspection conducted between May 17 and 21,1993, and May 25 to 28,1993, and June 7 to July 13,1993, one violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1993), the violation is listed below:

10 CFR Part 50, Appendix B, Criterion XI, states in part that "A test program shall be established to assure that all testing required to demonstrate that structures, systems and components will perform satisfactorily in service is identified and performed in accordance with written test procedures...."

Contrary to the above as of May 26,1993, motor-operated valve testing was not performed in accordance with written test procedures as evidenced by the following examples:

(1)

On May 16,1993, the torque switches on the motor-operated valve ICH-MOV142 were bypassed contrary to the requirement of Procedure 1/2 CMP MOVATS-1E, " Testing of Motor Operated Valves using MOVATS," Part D, Step 3.C and caused bending of the valve stem and other damages.

(2)

On May 19,1993, the valve ICH-MOV275C was overthrusted above 18,275 lbs., the limit specified in Procedure 1/2 CMP-75-MOVATS-1E, " Testing of Motor Operated Valves using MOVATS," during a test. The as left thrust of 19,525 lbs. was inaccurately dispositioned as less than 2% whereas the actual overthrust was 6.8%.

On May 26,1993, the torque switch setting for IFW-MOV151C was taken beyond 2.25, the maximum value specified in Procedure 1/2 CMP MOVATS-lE, " Testing of Motor Operated Valves using MOVATS," during a test and as a result the anti-rotation device on the valve stem was torqued beyond its limit.

l This is a Severity 1.xvel IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, Duquesne Light Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, NITN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region I, and a copy to the NRC Resident Inspector at the facility that is the A:BV931112. subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued to show cause why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
