NRC COMBINED INSPECTION REPORT NOS. 50-334/93-11 AND

50-412/93-12

This refers to the announced safety inspection conducted by Mr. M. Buckley of this office

from May 17 to 21,1993, and May 25 to 28,1993, on site with additional information

provided between June 7 and July 13, 1993. The findings of the inspection of this additional

information was forwarded to your staff on July 13, 1993. The inspection included a review

of activities authorized for the Beaver Valley Power Station in Shippingport, Pennsylvania.

At the conclusion of the onsite inspection, the findings were discussed with Mr. D. Spoerry

and other members of your staff.

Areas examined during the inspection are discussed in the enclosed report. The purpose of

this inspection was to followup on your activities, concerning Generic Letter 89-10, to

determine that motor-operated valves (MOVs) are being maintained in a condition that

ensures they will perform their function during accident conditions. MOVs play an

important role in protecting the health and safety of the public by assuring reliable operation

of safety-related valves during design-basis accident conditions. The inspection consisted of

an examination of the MOV testing program and a review of previously identified MOV

issues.

Based on the results of this inspection, certain of your activities appeared to be in violatiori

of NRC requirements, as specified in the enclosed Notice of Violation. A lack of test

control was evident in that ICH-MOV142 was overthrusted to the point where the stem was

bent, the anti-rotation device of 1FW-MOV151C was overtorqued during tests, and Nuclear

Engineering inaccurately dispositioning an administrative overthrust condition for 1CH-

MOV275C.

further NRC enforcement action is necessary to ensure compliance with NRC regulatory

requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and

its enclosures will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject to the clearance

procedures of the Office of Management and Budget as required by the Paperwork Reduction

Act of 1989, Pub. L No. 96.511.
