NRC INSPECTION REPORT NOS. 50-334/93-12 AND 50-412/93-13 AND

NOTICE OF VIOLATION AND NOTICE OF DEVIATION

This refers to the inspection conducted by Messrs. L. Rossbach, P. Sena, and S. Greenlee of

this office from May 11 to June 14, 1993. The inspection included a review of activities

authorized for the Beaver Valley Power Station in Shippingport, Pennsylvania. At the

conclusion of the inspection, the findings were discussed with Messrs. D. Spoerry and

G. Thomas and other members of your staff.

Areas examined during the inspection are identified in the report. The inspection consisted

of document reviews, interviews, and observation of activities important to public health and

safety. The purpose of the inspection was to determine whether activities authorized by the

licensee were conducted safely and in accordance with NRC requirements.

Based on the results of this inspection, cenain of your activities appeared to be in violation

of NRC requirements, as specified in the enclosed Notice of Violation (Notice). Also,

cenain of your activities appeared to deviate from your commitments, as specified in the

enclosed Notice of Deviation (Notice).

The violation involved the use of a heat-up curve that had been superseded by a recent

technical speciDeation amendment. Although no pressure and temperature limits were

exceeded, this is of concern because the curves used were less conservative than the

authorized curves and because the technical specification amendment was not properly

implemented.

The deviation involved not meeting commitments involving periodic testing of AMSAC.

This is of concern because of the potential for reduced system reliability to go undetected.

You are required to respond to this letter and should follow the instructions specified in the

enclosed Notices when preparing your response. In your response, you should document the

specific actions taken and any additional actions you plan to prevent recurrence. After

reviewing your response to these Notices, including your proposed corrective actions and the

results cf future inspections, the NRC will determine whether further NRC enforcement

action is necessary to ensure compliance with NRC regulatory requirements.

Also, your staff identified that a high radiation barrier was left open and that an improper

entry was made into a volume control tank cubicle. These violations were not cited because

of their minor safety significance, your prompt corrective actions, and because at the end of

,

this inspection period, you were preparing a response to other instances of violations of high

radiation area access control requirements. The continuing occurrence of these events is,

however, of concern.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and

its enclosures will be placed in the NRC Public Document Room.

The responses directed by this letter and the enclosed Notice are not subject to the clearance

procedures of the Office of Management and Budget as required by the Paperwork Reduction

Act of 1989, Pub. L No. 96.511.
