NRC INSPECTION REPORT NO. 50-498/93-21; 50-499/93-21 (NOTICE OF

VIOLATION)

This refers to the inspection conducted by Mr. J. 1. Tapia of this office on

May 10 through June 10, 1993.

The inspection included a review of activities

authorized for your South Texas Project, Units 1 and 2, facility. At the

conclusion of the inspection, the findings were discussed with you and those

members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of selective examinations of procedures

and representative records, interviews with personnel, and observations of

activities in progress.

The purpose of the inspection was to determine

whether activities authorized by the license were conducted safely and in

accordance with NRC requirements.

Based on the results of this inspection, certain of your activities appeared

,

'

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice).

The violation is of concern because it involved the

-

f ailure to ensure that the seismic qualification of the Qualified Display

Processing System (QDPS) was maintained by taking prompt and effective

corrective action to evaluate the effect of missing fasteners identified in

i

the card cages and power supply racks.

This condition, originally identified

-

by your system engineer on January 4, 1993, did not receive an evaluation for

system operability until April 29, 1993, after identification of the condition

by a member of the NRC Diagnostic Evaluation Team.

Your operability

evaluation subsequently determined that two Technical Specification governed

safety-related components, the Steam Generator Power Operated Relief Valves

and the Reactor Coolant System Subcooling Monitor, should have.been declared

inoperable as a result of these missing fasteners.

Thus, your failure to take

adequate corrective action resulted in potential violations of Technical

.

Specifications 3.7.1.6 and Technical Specification 3.3.3.6.

This violation was determined to be similar to previous violations concerning

(

your failure to take prompt and effective actions to correct problems. We

!

understand HL&P is already taking significant action to resolve weaknesses in

!

its corrective action program as a result of this and other significant

l

enforcement actions.

For this reason, no response to the Notice is required.

'

In addition to the violation, a previously identified unresolved item was

reviewed.

This unresolved item involved the operation of both units in

apparent violation of various Technical Specifications. - These conditions

,

resulted from errors in the methodology for adjusting the trip settings on

molded case circuit breakers and are of concern because inadequate maintenance

instructions resulted in 12 breakers, and subsequently the Technical

!

Specification governed motor-operated valves that were powered from these

breakers being considered inoperable.

At the end of this inspection an

engineering evaluation was being performed by your staff to determine if the

!

adjustments made to these circuit breakers were acceptable and the motor-

operated valves were operable.

This issue will remain unresolved pending

i

further NRC review.

In accordance with 10 CFR Section 2.790 of the NRC's " Rules of Practice," a
