SUMMARY

- HATCH UNITS 1 AND 2 This refers to the management meeting conducted in the Region II office on May 19, 1993. This meeting was held at our request to discuss operator performance and NRC's expectations involving the Unit I loss of shutdown cooling event on April 14, 1993 which is discussed in our Inspection Report No. 50-321 and 366/93-06. A brief summary, a list of attendees, and a copy of your handout is enclosed.

It is our opinion that the meeting was mutually beneficial. We will. evaluate the effectiveness of your corrective actions during future inspections.

In accordance with 10 CFR 2.790 of the NRC's " Rule of Practice," a copy of this letter and its enclosures will be placed in the NRC Public Document Room.

ENCLOSURE 1 MANAGEMENT MEETING

SUMMARY

On May 19, 1993, representatives from Georgia Power Company (GPC) met with the NRC in the Region II office in Atlanta, Georgia, to discuss operator performance involving the Unit 1 loss of shutdown cooling event on April 14, 1993.

Opening remarks were given by Mr. E W. Merschoff, Director, Division of Reactor Safety, Region II outlining the NRC's concerns and expectations about operator performance during this event.

GPC gave a presentation (Enclosure 2) on the issues. Mr. J. T. Beckham, Jr.,

Vice President, Hatch Project, provided opening remarks and introduced the presentation. The presentation was given by Mr. L. Sumner, General Manager, Hatch, and by Mr. J. D. Heidt, Manager, Nuclear Engineering 'and Licensing.

During the presentation issues were discussed in detail.

Following this,'the NRC closed the meeting.

A list of attendees at the meeting is contained in Enclosure-3.

EVENT 0740 Operator confinns that lEll-F015B is open during performance of ECCS status check, and SDC was.

functioning properly.

0745 PCIS mimic board being removed for hardened vent design change. Panel wires inadvertently grounded during removal, fuse blows.

Some electrical arcing is observed by work crew and operator passing by.

The mimic board, being heavy, is quickly laid face down on a special stand provided for the work.

0745+

Valve 1El1-F015B automatically closes.

U RHR pump continues to run but flow decreases to zero (not observed).

0745 - 0910 Loss of SDC went undetected.

Procedure 34GO-OPS-015-IS requires checking SDC flow and other parameters every four hours.

0910 Operator checked SDC flow as required by 34GO-OPS-015-IS and found it to be zero.

EVENT 0910+

Operator checked system alignment and found lEl1-F015B closed.

Operator attempted to open valve using control switch.

Valve cycled open and then closed.

RHR pump 1D was secured.

Fuel movement was stopped.

Loss of SDC procedure was~ entered.

0910-1015 Shift Supervisor initiated actions to get Maintenance to investigate and determme cause of valve closure.

1015 Fuse 1 A71-F22 was found to be blown and was replaced.

1043 Filling and venting of the RHR loop "B" was completed -

and SDC was placed in service.

PLANT RESPONSE

1. The bulk of the fuel remained in the Spent Fuel Pool, cooled by Fuel Pool Cooling.
2. The fuel in the vessel remained covered by approximately 52 feet of water.
3. There was no detectable increase in reactor coolant-temperature or Spent Fuel Pool temperature.
4. The operating RHR pump was not damaged.

LOSS OF S1WrDOWN COOLING HATCH UNTr 1

CORRECTIVE ACTIONS TAKEN An Operating Order was issued stating that the CBO will tour the control room front panels once every hour and will tour the back panels at-least every two hours. (4/16/93)

Training was conducted onLthe event for all Operations personnel on shift-addressing the needLto investigate abnormal occurrences. (4/23/93).

A letter was issued from the Manager of Operations to all Operations-personnel describing two specific plant events (including this event) and reminding personnel of the need to maintain a questioning attituc.e.

LOSS OF SHUTDOWN COOLING HATCH UNIT 1

CORRECTIVE ACTIONS TAKEN Placard added to the panel containing the mimic board that reads "When removing this panel to gain access to lights hidden by terminal strip bracket, care should be taken to avoid shorting the lower row oflights to the panel opening."

The operator had the core flow parameter displayed on the front panel that would show a change in core flow if the RHR flow to the i

vessel was isolated.
