- SUBJECT: NRC Inspection Report No. 50-293/93-10
An announced safety inspection of the radiological controls pmgram at your facility was
conducted by Mr. J. Noggle on May 10 - 14, 1993. The inspection findings were discussed
with members of your staff on May 14, 1993.

Areas reviewed during the inspection were important to health and safety and are discussed '
l

in the enclosed inspection report. These areas included review of: audits and surveillances,
collective personnel exposure status, outage radiological controls, and the implementation of
the internal and external exposure radiological programs during a refueling and maintenance -
outage,

It was apparent that significant resources were devoted to shielding various high radiation
sources to reduce personnel exposures and to your ALARA program. There appeared to be
,
a management commitment to reduce exposure to station workers and advanced planning was
evident. Notwithstanding, some program weaknesses in this area were identified.

Two violations were identified relative to an event on April 24,1993. The first involved
thme workers who entered a high radbtion area located in the "B" Quad without signing in
on the correct radiation work permit, and, as a result, did not receive appropriate instmetions
on the radiological hazards associated with the job. The second violation resulted when one
of the workers' alarming dosimeter began to continuously alarm but the worker did not leave '
,
the area as required. These violations are cited in the enclosed Appendix A. - You are
required to respond to the enclosed Notice of Violation and should follow the instmetions
contained therein.

In your msponse to the first violation, please also address the workers crossing the high-rad
rope boundary, which we understand was inadvertent. We would like your view as to
whether this was a msult of poor posting, whether the workers did not understand the

significance of the posting, or whether there was some other root cause. Please indicate your
corrective actions associated with this issue.

With regard to the second violation, as a result of our review of the written statements made
by the workers after the entry, we believe that your policy is not clear regarding what should
,
be done in the event a worker's dosimeter begins to continuously alarm. In addition, it is
our understanding that instmetions are given to workers in general employee training to leave
the work area when this occurs. We also understand that under some conditions workers are
expected to stay in their work area when their dosimeter either intermittently or continuously
alarms. In your response to this violation, please provide your station policy on this matter.
If corrective actions are warranted to ensure that workers clearly understand what actions
they are to take when their dosimeter begins to continuously alarm, please indicate what -
those corrective actions are.
Your staff generated 46 radiological problem repons for the first calendar quarter of 1993
and 79 radiological problem reports for calendar year 1992. The majority of these reports
were in the area of procedare violations and non-compliances with radiation work permit
requirements. While we applaud your program for identifying these issues, we am
concerned that your staff may not be taking effective actions to reduce the frequency of these
. problems. We would like to understand better the actions you may be planning to address
this issue. We would welcome the opportunity to meet with you and your staff to discuss the
issue further, or if you prefer, you may address the issue in writing. If you choose the
latter, we request that you do so in your response to this letter.
