. APPENDIX A NOTICE OF VIOLATION Boston Edison Company Docket No. 50-293 Pilgrim Station License No. DPR-63 As a result of an inspection conducted on May 10 - 14, 1993, the following violations of NRC requirements were identified. In accordance with the NRC Enfortement Policy (10 CFR 2, Appendix C), the violations are listed below':

10 CFR 19.12 requires, in pan, that all individuals working in a restricted area be instructed in the precautions and procedures to minimize exposure to mdioactive materials, in the purpose and fimetions of protective devices employed, and in the applicable provisions of the Commission's regulations and licenses.
Contrary to the above, on April 24,1993, three individuals made an unauthorized entry into a high radiation area in the "B" Quad, and had not been instructed in the radiological conditions in their work area and had not been instmeted in the precautions and pmcedures to minimize exposure to i
radioactive materials.

Technical Specification 6.11 requires that procedures for personnel radiation protection shall be approved, maintained and adhered to for all operations involving personnel radiation exposure. Procedure No. 6.1-022, Rev. 32,
" Issue, Use, and Termination of Radiation Work Permits," specifies that general RWPs shall not be used to enter high radiation areas except for operator rounds or for radiological protection entries (section 6.0 [4]).
Contrary to the above, on April 24,1993, three construction contractors made an entry into a high radiation area in the "B" Quad by using the general Radiation Work Permit No. 5011, which prohibits high radiation area entries.

the worker shall move to a lower dose rate area or mport to radiation pmtection personnel.
Contrary to the above, on April 24,1993, three individuals made an entry into a high radiation area in the "B" Quad. When one of the workers' alarming pocket dosimeter began to continuously alarm, the worker continued to work in the area, with his dosimeter alarming, until reaching a work termination point at which time he left the area.
This is a Severity Level IV violation. (Supplement VI)'
Pursuant to the provisions of 10 CFR 2.201, Boston Edison Company, is hemby required to submit a written statement or explanation to the U. S. Nuclear Regulatory Commission, ATrN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator Region I, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations; and (4) the date when full compliance will be achieved. If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked, or why such action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the msponse time.
