NOTICE OF VIOLATION The Detroit Edison Company Docket No.-50-341 Fermi 2 Power Station License No. NPF-43 During an NRC inspection conducted on March 29 through June 2, 1993, a violation of NRC requirements were identified.

In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions,"

i 10 CFR Part 2, Appendix C, the violation is listed below:

10 CFR Part 50, Appendix B, Criterion XVI, Corrective Action, requires, in part, that measures shall be established to assure that conditions adver,e to quality and nonconformances are promptly identified and corrected. The identification of the significant condition adverse to quality, the cause of the' condition, and the corrective action taken shall be documented.

Contrary to the above:

The licensee failed to promptly identify and correct the cause of a a.

i significant condition adverse to quality regarding a water hammer event which occurred on or before February 13, 1992, in the High Pressure Coolant Injection (HPCI) system. A piping failure analysis was not i

conducted until May 1993, approximately 15 months after the initial restraint deformation, b.

As of the end of the inspection period, June 2,1993, the licensee had failed to document the cause and corrective actions for a significant condition adverse to quality involving an increased level of water contamination in the HPCI lube oil system.

This is a Severity Level IV violation (Supplement I).

With respect to Part b., the inspection showed that actions had been taken to correct the identified violation and to prevent recurrence.

Consequently, no reply to the violation is required and we have no further questions regarding this matter.

With respect to Part a.,

pursuant to the provisions of 10 CFR 2.201, The Detroit Edison Company is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATIN: Document Control Desk, Washington D.C. 20555 with a copy to the U. S. Nuclear Regulatory Commission, Region III, 799 Roosevelt Road, Glen Ellyn, Illinois, 60137, and a copy to the NRC Resident Inspector at the Fermi 2 facility within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order or a demand for information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
