By letter dated May 24, 1993 the NRC issued a Notice of Violation as described in Inspection Report No. 50-269/93-13, 50-270/93-13, and 50-287/93-13.

w Violat on 269,270,287/93-13-01, Severity Level IV Technical Specification 6.4.1 requires that the station be operated in accordance with approved procedures.

Oconee Scaffolding Manual Section 3.C requires that scaffolds must not obstruct manual or automatic movement of equipment including valves.
Contrary to the above, scaffolding was built such that it blocked the movement of the positioner feedback arm on feedwater regulating valve 2FDW-32 controller and resulted in an error signal that caused the valvt to close.
This resulted in emergency feedwater actuation.
RESPONSE
1.
The reason for the violation, or if contested, the basis for disputing the violation:

l This violation occurred due to a personnel error on the part of the team erecting the scaffold at 2FDW-32.
The employee Installing the scaf fold platform was not f amiliar with all the moving parts of the valve and installed the platform under the i
positioner feedback arm.
2.
The corrective steps that have been taken and the results achieved:
The immediate corrective action was for the scaf fold builders to modify the scaf fold, thereby removing the interference with the positioner feedback arm.
This corrective action was i
performed after approval was given by Operations.
l The incident was entered into the Problem Investigation Process (PIP) and is documented on PIP 2-093-0359.
3.
The corrective steps that will be taken to avoid further violations:

l Mechanical Maintenance will issue a communication package for scaffold builders to ensure they are aware of moving parts on valves.
Included in the package will be a review of this event as well as valve sketches showing details of which parts move.
This package will be reviewed by September 1, 1993.
