SUBJECT: NOTICE OF VIOLATION
NRC INSPECTION REPORT N05. 50-528/529/530/93-11
This refers to the inspection conducted by J. Sloan, H. Freeman, A.
MacDougall, J. Melfi, F. Ringwald, and D. Kirsch of this office, on March 25
through April 26, 1993. The inspection included a review of activities
authorized for your Palo Verde facility. At the conclusion of the inspection,
the findings were discussed with those members of your staff identified in the
enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). One of the violations involved the failure to plug a
defective steam generator tube and is of particular concern because your
corrective actions from a previous occurrence were ineffective. This
violation is particularly significant due to the recent steam generator tube
rupture event at Unit 2.
You are encouraged to continue your efforts and
close management attention to identify the root cause of the tube rupture and
to identify other tubes which may contain defects. We will continue to
closely h.cnitor your progress in this area.
On May 18, 1993 the NRC staff issued a Notice of Enforcement Discretion
regarding the failure to meet the Technical Specifications surveillance
testing requirements for snubbers. This matter will be reviewed during a
future inspection.
Our inspection of your corrective actions program identified that some APS
personnel were aware that some Palo Verde personnel had not fully subscribed
to the APS problem identification philosophy and that this information had not

been brought to management's attention. We understand that you are evaluating
corrective actions for this issue. We will review your efforts during a
future inspection.

to the clearance procedures of the Office of Management and Budget as required
by the Paperwork Reduction Act of 1980, Pub. L. No. 96.511.
