t Office of Nuclear Reactor Regulation items of Interest Week Ending June 11, 1993 Cost Beneficial Licensina Actions (CBLAs)
On June 8, 1993, the staff approved VEPC0's requests regarding auxiliary feedwater pump inspections for North Anna 1 and 2 and Surry 1 and 2.
Rather than the previous prescriptive requirements, future inspections will be based on monitoring pump differential pressure and vibration, and trending and-predictive analysis of parameters such as wear metals, contaminants, additives, particulate count and viscosity of the lube oil.
The licensee states that its cost savings will amount to $190K per year in avoidance of unnecessary pump teardowns and inspections.
Seouovah Units 1 and 2 A Restart Panel meeting with TVA is scheduled for June 14, 1993, at Region II to discuss the status of the restart efforts.
TVA continues.to work off backlogs, perform secondary plant piping repairs resulting from secondary plant reliability and design studies, and implement plant modifications.
t Restart of Unit 2 is presently scheduled for late in July, and Unit 1 approximately one month later.
Unit I continues in its refueling outage, with fuel loading expected to start during the second week of June.
TVA announced that Ken Powers, Plant Manager at Peach Bottom, will become the new Plant Manager at Sequoyah effective June 28, 1993.
Thermo-Laa Fire Barrier Test Proaram NRC staff met with representatives of the Nuclear Utilities Management and Resources Council (NUMARC) on June 3, 1992, to discuss the concerns expressed in a May 4,1993, letter from J.. Taylor on the industry's Thermo-Lag fire barrier test program.
NUMARC provided the NRC with an outline and schedule of the industry's Thermo-Lag test program.
NUMARC expressed the intention to begin combustibility tests on June 9, 1993, and complete 17 fire endurance tests by November 1993.
NUMARC also expressed plans to conduct 3-hour barrier ampacity tests, although these tests are not yet scheduled.
NUMARC stated its intention to generically apply to the industry ampacity tests conducted by Texas Utilities Electric Company on 1-hour barriers.
NRC attendees provided feedback on the program and its schedule during the meeting and arranged a working-level meeting to discuss the details of the NUMARC program on June 28, 1993.
Salem 2 Control Rod Operation Problems Salem 2 (PSE&G), in restart from refueling, experienced numerous difficulties with the rod control and position indication instrumentation.
There were problems with group and individual rod position indication and in some cases i
dropping of groups into the core.
Maintenance had been performed on the system during reload and (because of the problems) during startup.

2 failures were found on the control rod electronic cards during trouble-shooting.
During tests, it was found that one of the rods had withdrawn further following an insert command.
PSE&G believed (6/4/93) the problems were corrected, but Region I initial inspection and review indicated a concern with the single rod withdrawal event since the root cause was not clearly established.
The Westinghouse Regulatory Response Group has been activated and is expected to meet with the staff next week. 'The Reactor Systems Branch is assisting the Events Analysis Branch in the preparation of an Information Notice.
The CE and B&W Owners Groups have been contacted about the problem.
Interim Voltage Based Pluaqina for Steam Generator Tubes The report of the special task group on interim voltage based plugging i
criteria for steam generator tubes has been approved by NRR and RES management and was placed in the NRC public document room on Monday, June 7.
The report provides the conclusions and recommendations of the special NRR/RES task group that was formed in February to review the acceptability of the interim voltage based criteria for outside diameter stress corrosion cracking at steam generator tube to tube support plate intersections.
Public comments on the report will be solicited.
Following evaluation of public comments, the report will serve as the basis for development of an interim generic position on the voltage based plugging criteria.
Operator licensino Generic Fundamentals Examinations On June 9, 1993, the Operator Licensing Branch administered the generic fundamentals section of the initial written licensing examination to 142 prospective operator and senior operator license applicants at 24 facilities.
The examination, which is administered in BWR and PWR versions and lasts 2 and 1/2 hours, includes questions on reactor theory, thermodynamics,.and plant components.
This examination is a prerequisite for taking the site-specific written examination and operating test, which focus on plant systems, operating procedures, and administrative requirements.

Office of Nuclear Material Safety and Safeguards Items of Interest Week Ending June 11, 1993 Prairie Island Independent Spent Fuel Storaae Installation On May 28, 1993, the Minnesota Court of Appeals ruled that the Minnesota Legislature must vote to approve the Independent Spent Fuel Storage Installation (ISFSI) at the Northern States Power Company's (NSP) Prairie Island site near Red Wing, Minnesota.
The ISFSI is opposed by the Mdewakanton Sioux Indian tribe, who previously had an interest in having the Monitored Retrievable Storage facility on their land.
lhey argued that the ISFSI would be permanent and, therefore, under the Minnesota Radioactive Waste Management Act, the Legislature must approve it.
NSP may now appeal the decision to the Minnesota State Supreme Court or wait until the Legislature reconvenes in February 1994. It would take four to ten months before the State Supreme Court could hear the case.
Contingent on issuance of a 10 CFR Part 72 license, NSP had planned to load its first cask early in 1994.
Meetinas Regardina Spent Fuel Transport Casks Beina Developed for DOE Transportation Branch staff participated in two meetings with the Department of Energy (DOE) and its contractors to discuss the design of three casks being developed for transport of spent fuel under the Nuclear Waste Policy Act. On June 2, 1993, General Atomics Company (GA) discussed the Model No. GA-4 and GA-9 casks.
These are legal weight truck casks which carry 4 PWR or 9 BWR assemblies.
The discussion centered on the design of the GA-4 fuel basket and on the scale model testing program.
GA plans to submit applications for Nuclear Regulatory Commission certification of the GA-4 and GA-9 in January 1994.
On June 3,1993, Babcock & Wilcox Fuel Company (B&W) discussed the Model No. BR-100 cask.
The BR-100 is a 100-ton rail / barge cask which can carry 21 PWR or 37 BWR assemblies.
The discussion centered on design changes made to the fuel basket and scale model testing of the package.
B&W plans to submit an application for the BR-100 in January 1995.
i Nuclear Reaulatory Commission / North Carolina Meetina on Performance Assessment On June 4, 1993, members of the Division of Low-Level Waste Management and Decommissioning, Offices of State Programs and Nuclear Regulatory Research met with representatives from the North Carolina Division of Radiation Protection (DRP) and their consultants.
The meeting was held to discuss new information and work-in-progress on the Nuclear Regulatory Commission's performance assessment methodology for low-level waste (LLW) disposal facility siting.
The meeting also provided an opportunity to discuss NRC staff comments on the DRP's License Application Review Management Plan. The NRC staff provided the requested technical assistance to the North Carolina state regulatory agency personnel, and the DRP supplied another perspective on LLW siting issues.

2 Nuclear Fuel Services low Enriched Uranium Amendment During the week of June 7, 1993, the staff of the Office of Nuclear Material Safety and Safeguards, Division of Fuel Cycle Safety and Safeguards approved an amendment to Nuclear Fuel Services (NFS) License SNM-124 to authorize the operation of the Low Enriched Uranium (LEU) Plant at the Erwin, Tennessee site. Operation of this plant was not authorized under the May 7, 1992, approval of NFS' timely renewal application because of insufficient information and an NFS decision to defer on requesting approval in light of the then existing business climate.
The LEU Plant would be utilized during the NFS downblending of high enriched uranium received from Russia to produce enrichments useful in light water reactors.
It is the staff's understanding that NFS intends to submit another amendment request to authorize the use of higher uranium concentration solutions in the LEU Plant.
Meetina with Nuclear Fuel Services On June 4, 1993, representatives of the Division of Fuel Cycle Safety and Safeguards met with Nuclear Fuel Services (NFS) management.
The purpose of the meeting was to discuss the potential impact of physical protection regulations upon NFS in the event that all site processing activity ceases.
At issue was the minimum physical protection staff levels required to maintain compliance with 10 CFR 73.46.
NFS will submit a proposed change to their physical security plan addressing this issue.
Safeauards Technical Bilateral Discussions Regulatory and International Safeguards staff is to participate in safeguards technical bilateral discussions between the United States and Japan (during the week of June 14) and Australia (during the week of June 21) in Tokyo and Canberra respectively.
Nuclear Regulatory Commission topics include the implementation of International Atomic Energy Agency safeguards in the U.S.,
criteria for the termination of IAEA safeguards on nuclear materials contained in waste, and some of the United States activities relating to strengthening measures for IAEA safeguards.
International Atomic Enerav AaencV/Kazakhstan Sponsored Seminar for NIS on the "Oraanization of State Systems of Accountina and Control" The International Atomic Energy Agency and the Atomic Energy Agency of the Republic of Kazakhstan are sponsoring a seminar for the benefit of the Newly Independent States (NIS) on the " Organization of State Systems of Accounting and Control" (SSAC) during the week of June 14, 1993, in Alma Ata, Kazakhstan.
The Chief of the Regulatory and International Safeguards Branch will be participating in the seminar as an invited speaker. The subject of the presentation is "The United States Nuclear Regulatory Commission Program for Material Control and Accounting and Physical Protection Regulation."
Revised Procedural and Pro.iect-Soecific Aareements The Division of High-Level Waste Management (HLWM) and the Department of Energy (DOE) have revised the " Procedural Agreement Between the Nuclear JUNE 11, 1993 ENCLOSURE B
3 Regulatory Commission and the U.S. Department of Energy Identifying Guiding Principles for Interface During Geologic Site Investigation and Site Characterization" (Procedural Agreement), and the " Agreement Between the U.S.
Department of Energy Office of Civilian Radioactive Waste Management and the Nuclear Regulatory Commission Division of High-Level Waste Management During Site Characterization Programs and Prior to the Submittal of an Application for Authorization to Construct a Repository" (Project-Specific Agreement).
These documents outline the revised procedures for staff consultation and exchange of information which HLWM, DOE, and its designated contractors will observe in connection with the site characterization phase of the high-level waste program.
The substantive changes that were incorporated into the Procedural Agreement dealt with newly added or revised guidelines for conducting technical exchanges, site visits, licensing and management meetings, and quality assurance audits and surveillances.
Similarly, major changes in the Project-Specific Agreement included revised guidelines for preparing interaction reports, maintaining and distributing site characterization data, communications between points of contact from NRC and DOE project offices, acquisition of samples during site characterization activities by NRC contractors, and specific on-site representative responsibilities and authority.

Office for Analysis and Evaluation of Operational Data Items of Interest Week Ending June 11, 1993 Diagnostic Evaluation and Incident Investigation Branch (DEIIB)
The South Texas Project diagnostic evaluation team report was forwarded to the ED0 and the report is in the process of being issued.
Technical Trainino Center (TTC)
The pilot Nuclear Criticality Safety Course (H-lll) was conducted at the Technical Training Center June 7-11, 1993.
The course was developed by the TTC through a contract with the Oak Ridge Institute for Science and Education (0 RISE) with technical support from NMSS.
Seventeen students representing NMSS, AE00, RES, OE, Ril, Rlll and RV attended the course which was presented by personnel from ORISE and a consultant, Dr. Ronald Knief.
Course topics included the background of nuclear criticality safety, regulatory requirements, nuclear theory, nuclear criticality safety principles, fuel facility operations and historical accidents.
Additional courses are planned.
after a review of the pilot course.
Incident Response Branch (IRB)
IRB Branch Chief was in Paris, Irance to report on U.S. participation in INEX-1, the international tabletop exercise to examine communication processes between neighboring countries in the event of a nuclear accident.
IRB staff provided training for DOE Federal Radiological Monitoring and Assessment Center (FRMAC) responders in RTM-92 and the NRC's response role and procedures during an emergency.
IRB staff provided training to Pegion IV staff participating in the FRMAC-93 radiological exercise.
Reactor Operations Analysis Branch (ROAB)
On June 2,1993, Hal Ornstein attended a meeting of the Air-Operated Valve Users Group.
He made presentations on air-operated valve (A0V) operating data and observations made during his trip to Germany (December 1992), including a discussion of German requirements for performance testing of motor-operated valves, solenoid-operated valves, and A0Vs.
On June 10, 11 Hal Ornstein attended an EPRI/NMAC Instrument Air Workshop.
He made a presentation on Instrument Air Systems Operating experience.
The Accident Sequence Precursor (ASP) events found by ORNL in 1992 operational data have been published in draft and have been distributed within the NRC.
There were 27 precursor events found in calendar year 1992, and these events are being sent via the PMs to the licensees for comment.
Comments are being requested on the adequacy of the ASP modeling of the plant and. plant.
equipment, and also on the reasonableness of the analysis assumptions regarding recovery of failed or degraded equipment.
Comments are requested by June 30, 1993.

i Fred Manning of the Reactor Operations Analysis Branch /AE00, will participate in a seminar at Electricite de France (EDF) in Paris, France, providing EDF personnel with a fundamental understanding of the NRC Accident Sequence Precursor Program (ASP). This will assist EDF in establishing the same type of program to evaluat, nuclear plant operational data using a risk based method.
The US NRC started the ASP program in 1979 following the TMI accident.
We expect some level of sharing information between NRC and IDF.
Trends and Patterns Analysis Branch (TPAB)
AE00 sponsored a training program for the use of the Sequence Coding Search System (SCSS) database.
A user friendly " Front End Processor" that operates on a personal computer has been developed to enable NRC staff to access and initiate queries to the database via a menu driven selection screen.
A-training course on the use of this program has recently been completed by 28 members of the NRC staff from NRR, RES, and AE00.

Items of Interest Week Ending June ll, 1993 GSA Deleoation Review A team of General Services Administration (GSA) program specialists was on-site the week of June 7.
The purpose of the visit was to review and evaluate the Division of Contracts and Property Management's facility management program with respect to the responsibilities which GSA has delegated to the agency for the operation, maintenance, and management of NRC facilities.
GSA performs a review of delegated agencies every two years. The GSA team has conducted thorough inspections of NRC buildings, performed critical reviews of program documentation, and held in-depth meetings with NRC personnel responsible for carrying out the elements of the building delegation program, i
The GSA team will provide the staff with a summary of their review at a meeting to be scheduled within two weeks.
Their final written report will be submitted within 30 days.
New Executive Order (E.0.) for National Security Information The NRC Office of Administration (Security) is represented on the interagency Task Force created by the Presidential Review Directive 29. The Task Force, chaired by the Director, Information Security Oversight office, is responsible for drafting and proposing to the National Security Council no later than November 30, 1993, a security classification system to replace the existing E.0.12356, " National Security Information." Task Force Subcommittee members have been interviewing knowledgeable and interested Government officials and members _ of the public.
Oral presentations were heard at public meetings on June 9 and 10, 1993.
NRC is specifically represented on the Subcommittee on Duration.
That Subcommittee is addressing the question: What. steps can be taken to declassify information as quickly as possible?

Office of Information Resources Management items of Interests Week Ending June 11, 1993 Electronic Transfer of LERs and MORs Workina Group The Office of Information Resources Management (IRM) staff met with the Office for Analy. sis and Evaluation of Operational Data (AE00) task manager to resume the Electronic Transfer of Licensee Event Reports (LERs) and Monthly Operating Reports (MORs) Working Group.
The group is exploring the use of more modern technologies to transfer operating and event data from licensees to the NRC. The group discussed the roles that IRM and AE00 should play in the effort and decided that a formal agreement outlining each organization's responsibilities should be developed.
The group also visited the Department of Energy (DOE) to observe their automated occurrence reporting system (ORPS), a system for transmitting, updating, approving, storing, distributing and analyzing occurrence reports from DOE facilities.
The working group members were favorably impressed with DOE's system and would like to see many of the ORPS features incorporated into a similar system for LER reporting, particularly the user-friendly menus and screens.
In addition, IRM staff updated the AE0D task manager on the dissemination of the IRM developed Wordperfect LER template that has been provided to eighteen licensees.
Computer Security The Office of Information Resources Management recently published NUREG/BR-0168,
" Security Policy for Processing and Handling Sensitive Unclassified Information in the AUTOS / Local Area Network Environment,"
which provided procedures for handling and processing all categories of sensitive unclassified information (i.e., Official Use Only, Proprietary, and Unclassified Safeguards) on the AUTOS /LAN.
The NUREG will be distributed to all NRC employees, provided to students in the ITS Training Lab, and given to all new employees during the new employee orientation program.
Destruction of Merit Promotion Files On June 3, 1993 The Office of Information Resources Management (IRM) was informed by staff from the Office of Small and Disadvantaged Business Utilization and C.vil Rights and the Office of the General Counsel that all class action suits are now resolved.
IRM will inform the Office of Personnel's Records Liaison Officer of this finding and will request authorization to destroy the 424 cubic feet of subject records currently stored at the Archival Facility.

Office of Personnel Items of Interest Week Ending June 11, 1993 1AG/CODAT Meetina Attended On. lune 9, Peter J. Goldman, Associate Director for Training and Development, attended the Steering Committee meeting of.the Interagency Group Committee on Development and Training (IAG/CODAT).
Items discussed included the status of efforts to revise training regulations, the results of a meeting of Federal Personnel Directors with the new Director of the Office of Personnel Management, and input to Vice-President Gore's National Performance Review.
Alcoholism Conference Attended Pat Kaplan, Employee Assistance Program Manager, attended the Northeastern Conference on Alcoholism and Drug Dependence, held May 23-26, 1993, in Newport, Rhode Island.
The objective of the conference was to provide attendees with the latest research findings and recent developments in the diagnosis and treatment of alcoholism and drug addiction, codependency, eatin'g and sleep disorders, and AIDS. Other topics included spiritual values and.
emotional health, drug testing, and managed health care. Sponsors were Edgehill Newport Treatment Center and the American Society of Addiction Medicine.
Wellness / Fitness Center Plannina Under Way On June 7, Henry Rubin, Fitness Center Program Manager, and Patricia Kaplan, Employee Assistance Manager, conducted their first meeting with the fitness consultant hired to design and develop the Wellness / Fitness Center planned for White Flint Two.
To assist with the design and development, including equipment layout, Henry Rubin visited the Department of Agriculture's Fitness Center on June 9.
SES Guidance The new SES Employment and Staffing Directive, MD 10.135, has been published.
A copy was provided to each SES member through normal directive distribution.
This directive provides guidance for succession planning, SES Candidate Development and a variety of employment and staffing activities.

Office of Enforcement Items of Interest Week Ending June 11, 1993 Sionificant Enforcement Actions A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $250 was issued on June 8, 1993, to Steel Warehouse Company, Inc., South Bend, Indiana.
Steel Warehouse is generally licensed to use byproduct material contained in thickness measurement devices.
The company has 3 fixed gauging devices containing Am-241. When one of them was damaged, the company

shipped it back to the vendor in a cardboard box in violation of several DOT transportation requirements for labelling and packaging.
Collectively, these violations constituted a Severity Level III violation of 10 CFR 71.5(a).
The base civil penalty is $500 and this was mitigated 50% for corrective actions.
None of the other factors were applied.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $6,000 was issued on June 9, 1993 to Mayo Foundation, Rochester, Minnesota The action was based on the willful failure of a researcher to perform contamination surveys when working with phosphorous-32. As a result, contamination was spread off-site to a church, private automobiles, clothing, and homes.
A second violation involves inadequate surveys to detect the extent of off-site contamination.
The two violations are classified in the aggregate as a Severity Level II problem.
Mitigation was applied because the licensee identified the fact that the researcher failed to perform-contamination surveys.
Escalation was applied because the failure' occurred on more than one occasion.

A Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $2,500 was issued on June 11, 1993, to Cassia Memorial Hospital, Burley, Idaho.
The action was based on a Severity Level III problem involving the licensee's failure to establish a written Quality Management Program and 10
. additional violations.

The base civil penalty for a Severity Level III violation is $2,500.

Second Buildina Status During the week, at OWFN ten new trees were planted in the front of the building and new top soil was placed inside the new planters.
Irrigation piping installation continued at the rear of OWFN.
At TWFN tree and shrub planting and irrigation piping installation continued.
Preparations were underway for remaining sidewalks not yet poured and a sidewalk ramp on the east side of the plaza at the location of the Plaza level connector between the two buildings.
Also during the week, inside TWFN the parking garage levels A through D were cleaned and stripes were painted on the floors.
Ceilings in the garage areas on P1 (A level) under the occupiable portions of the first floor were completed as were ceilings on P3 (C level) under the auditorium floor.
Cleaning of floors two through ten is in progress.
Air handling units, chillers and main cooling towers were tested. The outside stair rail between the TWFN plaza and the P1 auditorium entrance was installed during the week.
On June 5 and 7, CONS staff reviewed addendum drawings for the TWFN interior construction and provided comments to GSA and the developer.
On June 7, the Director met with the ADM Director, OP Director and their staffs to discuss safety and health issues related to the P1 pedestrian connector between the two buildings.
On June 8, CONS staff met for the second time with the National Institute of Science and Technology project staff working on the indoor air quality testing protocol at TWFN.
The staff toured the TWFN building shell.
On June 9, the developer's representatives announced that the County permit review of construction drawings by the Department of Environmental Programs had been set back by the Fire Marshall review which could cause a delay in the start of work on the interior. Also, the developer promised to provide a schedule from OMN1 to the government by the week's end.

Office of State Programs Items of Interest Week Ending June ll, 1993 Duke Power Government AaenCY Trainina Robert Trojanowski, Region 11 State Liaison Officer, participated in the Duke Power Company annual training program for State and local elected and appointed officials on June 8,1993 in Charlotte, North Carolina.
Mr. Trojanowski discussed the NRC emergency response organization, Price-Anderson, the Stafford Act and the NRC response to Hurricane Andrew.
Yankee Rowe Public Meetina Marie Miller, Region 1 State Liaison Officer, attended a public meeting on decommissioning issues associated with Yankee Rowe on June 9, 1993 in Buckland, Massachusetts.
The Yankee Atomic Electric Company has. proposed removal of certain plant components (four steam generators, the pressurizer and reactor vessel internals) prior to NRC approval of the decommissioning plan.
The purpose of the meeting was to obtain public comment on the proposal.
Representatives from the Commonwealth of Massachusetts participated l
in the meeting.
Subcommittee on Water Resources Meetina Maria Lopez-Otin, Federal Liaison, attended the Subcommittee on Water Resources (SWR) meeting on June 9, 1993.
The SWR was briefed on the work of the Interagency Coordinating Committee on Oil Pollution Research.
Given the need to coordinate research in the Federal government, the SWR also heard a presentation on the grant awarding process at the National Science Foundation in the Hydrological Sciences area. During the SWR's business session, there was much discussion of the philosophical underpinnings of the report the SWR l
has been writing, "A U.S. Water Resource Science Strategy".
The SWR decided that it did not have a sufficiently clear mandate to develop a government-wide strategy.
It voted change the concept and the term " strategy" was dropped from the title.
This vote was consistent with NRC's views.
The SWR also-requested that each agency representative begin working with their principals on how to best prepare the retroactive inventory regarding water resources research that will be needed.
Watts Bar Emeroency Preparedness Robert Trojanowski, Region 11 State Liaison Officer, participated in a Regional Assistance Committee (RAC) meeting on June 10-11, 1993 in Atlanta, l
Georgia. The purpose of the meeting was to conduct a comprehensive review of the site specific State and local emergency plans for the Tennessee Valley Authority Watts Bar nuclear facility to ensure conformance with NUREG-0654.
The RAC had reviewed and endorsed the Watts Bar emergency plans approximately eight years ago. A full scale exercise is tentatively scheduled to be conducted in October 1993.

Office of International Programs items of Interest Week Ending June 11, 1993 foreion Visitort On June 8 and 9, Dr. Samuel Harbison, Chief Inspector of the UK Nuclear Installations Inspectorate, met with the Commissioners and staff to discuss a number of safety topics of mutual, current interest.
The topics included one-step licensing, assessment of older reactors, advanced designs and the certification process, fuel cycle and enrichment regulation and international safety issues.
Also, Dr. Harbison visited the Region V office and the Diablo Canyon power plant.

Region I Items of Interest Week Ending June 11, 1993 1.
Paine Yankee Partial-Participation Emeraency Preparedness Exercise A partial-participation emergency preparedness exercise was held at the Maine Yankee Nuclear Power Plant from 8:00 a.m. to 1:00 p.m. on 6/3/93.
The exercise involved escalation to a Site Area Emergency due to simulated reactor vessel head seal leakage and a partial loss of off-site power.
Preliminarily, licensee performance was very good, with no exercise weaknesses identified and. comprehensive briefings of the operating crew noted strength.

Indian Point 3 The Chairman toured the Indian Point 3 Nuclear Power Plant on Monday, June 7.
Tim Martin, Region I Administrator, accompanied the Chairman.
In addition, Congressman Fish from the State of New York and'several members of his staff, as well as Chairman Murray from the Energy Office',
State of New York, and members of his staff along with State Assemblywoman Sandra Galef participated.
Richard Flynn, Chairman of the NYPA Board of Trustees, was also onsite to participate.in the tour.
At the conclusion of the visit, a press conference was held.

Salem 1 Region I dispatched an AIT to Salem 1, late Friday, June 4, to investigate a possible single failure, which could cause a single control rod within a control rod bank to withdraw when a signal is received to insert the control rod bank.
The failure, which has been characterized as plausible by Westinghouse, was identified by a Region I inspector who was following up on a series of control rod-related problems at Salem 1.
The licensee has placed Unit 1.in cold shutdown and has developed a JC0 for Unit 2.
The AIT will investigate the series of failures leading up to the identification of the single failure and issues related to them.
NRR will follow up on the generic aspects of the single failure issue with Westinghouse. A representative from the New Jersey Department.of Environment & Energy is also observing the AIT.

Peach Bottom Don Miller, Vice President, Peach Bottom, announced that Ken Powers, Plant Manager, was resigning from Philadelphia Electric Company (PECo) effective June 25, 1993.
Powers came to Peach Bottom in September 1991 as Plant Manager. Ken Powers will become the Plant Manager for the Sequoyah Station, in Soddy Daisy, Tennessee.
It is expected that the new Plant Manager will be announced sometime in the next two weeks.

Region II Items of Interest Week Ending June 11, 1993 1.
Enforcement Conference On June 7, representatives from the Scientific Inspection Technology, Inc.
of Hixson, Tennessee were in the Region II Office to attend an Enforcement Conference regarding the violations that occurred during a radiography event on April 16, 1993, at a temporary job site in Salem County, New Jersey.

Meetina with State of Georaia and the EPA June 7, a Division of Radiation Safety & Safeguards Branch Chief met with representatives of the State of Georgia and the EPA to discuss the burial of radioactive material by the U. S. Air Force at Robins Air Force Base in the 1950s and 1970s. Analyses done of the burial material has shown that the only radioactive material in the burials is radium, which is no.t within NRC regulatory authority.
The burials also appear to contain hazardous waste and thus may be mixed waste. During the meeting, the-EPA and State agreed to coordinate actions regarding the cleanup of the site.
The State concluded that they had full authority to require cleanup of the site.
There was agreement to meet again in approximately six weeks to i
review progress.

Carolina Power & Liaht Company - Brunswick On June 9, the Regional Administrator and members of the Regional and NRR staffs attended a public Meeting at the Brunswick facility regarding restart schedule for Unit 1.

Enaineerina Manaaers' Meetina On June 8, the Regional Administrator and the Director, Division of Reactor Safety attended the Region !! Engineering Managers' Meeting at the Stouf fer Concourse Hotel at the Atlanta Hartsfield Airport.

Operational Safeauards Response Evaluation - H. B. Robinson On June 7-11, NRR conducted an Operational Safeguards Response Evaluation at the Robinson site.

Babcock and Wilcox (B&W) NNFD - Loss of Off-Site Power i
At approximately 5:00 p.m., on Friday, June 4,1993, severe weather in the Lynchburg, Virginia area caused a loss of off-site power to the plant for approximately eight hours.
The licensee activated their E0C and implemented their severe weather plan.
All_ SNM was secured in safe storage and/or configurations.
An emergency diesel generator _ provided emergency power to vital systems such as Security, NCS monitor and alarm system, effluent scrubbers, etc.
Although there was extensive damage to homes and businesses in the Lynchburg area, B&W NNFD did not suffer any JUNE 11, 1993 ENCLOSURE R

major problems. The licensee terminated operations on Friday and secured the facility for the weekend. Limited restart of activities was initiated at about 11:00 p.m. on Sunday, June 6, 1993, primarily those associated with the machine shops and a cleaning operation.
The other two facilities, B&W Fuel Company and B&W Lynchburg Technology Center, which are located on the same site were shutdown at the time of the storm.
The resident inspector responded to the site on Friday night and also observed the major restart of processes on Monday' evening.

On June 10 and 11, 1993, the Regional Administrator, Region III, visited the Dresden Station in Morris, Illinois.
During his visit, he met with the Dresden resident staff to discuss current issues and plant performance, in addition, he met with the Site Vice President and other-Station Management personnel.

Perry Nuclear Power Plant On June 7 and 8, 1993,.the Regional Administrator, Region III, visited.

During the visit, he made an unannounced tour of the plant the evening of June 7 and talked with reactor operators.
On June 8, he met with various members of the plant staff, including the Vice President - Nuclear, to discuss current plant performance staffing issues.

Cleveland Ohio Area On June 7, 1993, the Regional Administrator, Region III and a member of his staff visited the Cleveland Ohio area and toured:
(1) the Southerly Wastewater Treatment Plant; (2) the Chemetron former factory site-and Bert Avenue dump site; and (3) the Advanced Medical systems, Inc source manufacturing facility.
During the tours, the Region 01 Administrator examined the facilities, evaluated the safety and environmec.t:1 hazards,-
and discussed NRC concerns with facility representatives.

Open Enforcement Conference with Wolf Creek Nuclear Operatina Corporation (WCNOC)
The President and Chief Executive Officer of WCNOC and members of his staff met in the Region IV office on Wednesday, June 9, 1993, for an enforcement conference which was open to public observation.
The purpose of the enforcement conference was to discuss apparent Technical Specification violations stemming from an event at WCNOC. where, during plant startup, the plant changed reactor modes with both motor-driven auxiliary feedwater pumps inoperable due their operating handswitches being in the pull-to-lock position.
