NRC INSPECTION REPORT N05. 50-528/93-03, 50-529/93-03, AND 50-

530/93-03

During the period of January 11-15 and February 10-12, 1993, Mr. R. Bocanegra

of this office conducted an inspection of your Palo Verde Nuclear Generating

Station. The purpose of the inspection was to review the circumstances that

led to a radiation exposure to a worker which exceeded regulatory limits. The

findings of the inspection were discussed with Mr. W. Ide on January 15, 1993,

and Mr. Pat Hughes on February 12, 1993.

Areas examined during this inspection are described in the enclosed report.

Within these areas, the inspection consisted of selective examinations of

procedures and representative records, interviews with personnel, and

observation of activities in progress.

Based on the results of this inspection, four apparent violations were

identified and are being considered for escalated enforcement action in

accordance with the " General Statement of Policy and Procedure for NRC

Enforcement Actions" (Enforcement Policy), 10 CFR Part 2, Appendix C.

The

apparent violations included 1) an exposure in excess of dose limits in 10 CFR-

20.101(a); 2) failure to perform a survey in accordance with 10 CFR 20.201(b);

3) failure to follow radiation protection (RP) procedures; and.4) failure to

,

supply appropriate dosimetry as specified in 10 CFR 20.202(a). Accordingly,

'

no Notice of Violation is presently being issued for these inspection

findings. The number and characterization of apparent violations described in

!

the inspection report may change as a result of further NRC review.

An enforcement conference to discuss these apparent violations has been

I

scheduled for March 16, 1993. The purposes of this conference are to discuss

the apparent violations, their causes and safety significance; to provide an

opportunity to point out any errors in our inspection report; to provide an

opportunity for you to present your proposed corrective actions;-and to.

discuss other information that will help us determine the appropriate

enforcement action in accordance with the Enforcement Policy. You will be

advised by separate correspondence of the results of our deliberations on this

matter. No response regarding these apparent violations is required at this

time.

Additionally, a deviation from your UFSAR commitments was identified. The

deviation relates to calibration of containment high range radiation monitors,

as specified in the enclosed Notice of Deviation (Notice). You are requested

to respond to this deviation and should follow the instructions specified in

the enclosed Notice.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and the enclosure will be placed in the NRC Public Document Room.
