NRC OPERATIONAL EVALUATIONS REPORT NO. OL-50-324/90-02

This refers to the Nuclear Regulatory Commission (NRC) observation of crew

performance on the Brunswick simulator conducted by C. Casto during the period

May 18-19,1990.

The evaluation included a review of activities authorized

for your Brunswick facility. At the conclusion of the Operational Evaluations,

the findings were discussed with those members of your staff identified in the

enclosed report.

During the weeks of April 30 - May 7,1990, requalification examinations were

conducted at the Brunswick site in accordance with NUREG-1021

ES-601. An

unsatisfactory program evaluation was determined when three out of four crews,

and 14 out of 20 operators were judged to be unsatisfactory (Examination Report

OL-50-325/90-01).

In response, a meeting with NRC management was held in

Atlanta on May 15, 1990. As a result of that meeting, and in accordance with

NUREG-1021, ES-601, Operational Evaluations were scheduled.

C. Casto led a team which observed the Operational Evaluations on the simulator

during May 18-19, 1990.

It was found that four of four crews performed

unsatisfactorily during these evaluations. Performance deficiencies were noted

in the areas of command / control and communications, manipulative and cognitive

skills, and use of Emergency Operating Procedures.

Your attention is invited to the Details section of this report that incicates

significant operator weaknesses as a result of training program deficie0cies.

As discussed in the management meetings held in Atlanta during May 1590,

Carolina Power and Light (CP&L) will present a long term corrective action plan

to the NRC by June 30, 1990.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of-Federal Regulations, a copy of this letter and its enclosure

will be placed in the NRC's Public Document Room.
