1. NARRATIVE

SUMMARY

OF OPERATING EXPERIENCES Limerick Unit 1 began the month of April at 100% of rated thermal power. On April 5, reactor power was reduced to 90% to accommodate main turbine control valve testing. On April 19, the '1B' condenser waterbox was removed from service for condenser tube leak repairs, On April 21, reactor power was reduced briefly to 08% due to turbine bl0h back pressure. On April 23, reactor power was again reduced briefly 1o 94% due to turbine high back pressure. On April 24, the '1B' condenser waterbox was retumed to service. Limerick Unit 1 ended the month of April at a nominal 100% of rated thermal power.

Operational events that occurred during the month of April included:

On April 5, the main control room was manually isolated when a high toxic chemical concentration alarm was received in the control room. Following investigation, it was determined that the alarm was caused by a failed 'B' toxic gas monitor. The monitor was removed from service and repaired. After the monitor was returned to service, the main control room isolation was reset. On April 6, another high toxic chemical concentration alarm was received. The main control room was manually iso-lated until It was determined that the 'B' toxic gas monitor had caused another spurious alarm. Subsequently, the 'B' detector was replaced, the filter servo motor was lubricated, and the monitor was retumed to service. The 'B' toxic gas monitor has operated satisfactorily following the repair.

On April 20, the 'B' ESW loop was found to be inoperable when the 'B' ESW pump discharge check valve was found partially open and could not be closed. The check valve had been previously blocked for maintenance work and had been incorrectly reassembled. The Inoperable condition was found during post maintenance testing.

The condition was corrected immediately.

On A'.ril 27 a one hour notification was made to the NRC when it was discovered that tne offsite sirens could.nct beJnterrogated due to a.tadio signal irder. l ference. It was later verified that the sirens could be activated if required.

The source of the interference was found and eliminated. On April 29, the problem was resolved and the cirens were retumed to normal.

11. CHALLENGES TO MAIN STEAM SAFETY REllEF VALVES There were no challenges to the Main Steam Rollet Valves during the month of April.

1. NARRATIVE

SUMMARY

OF OPERATING EXPERIENCES Umerick Unit 2 began the month of April in OPCON 4. The unit had been shut down during the month j of March due to an Electrohydraulic Control system oil leak and reactor coolant conductivity problems as a result of a main condenser tube leak. On April 2, the unit entsrod OPCON 2 at 2017 hours0.0233 days <br />0.56 hours <br />0.00333 weeks <br />7.674685e-4 months <br /> and criticality was achieved at 0142 hours0.00164 days <br />0.0394 hours <br />2.347884e-4 weeks <br />5.4031e-5 months <br /> on April 3. OPCON 1 was entered at 1340 hours0.0155 days <br />0.372 hours <br />0.00222 weeks <br />5.0987e-4 months <br /> on April 3, and the main generator was synchronized the same day. Power ascension was held at 80% due to feedwater -

chemistry limitations. On April 6, reactor power was increased from 80% to 100% when feedwater chemistry reached acceptable limits. On April 10, reactor power was reduced to 15% to accomodate a control rod pattern adjustment. On April 23, rasetor power was reduced to 80% in response to increasing reactor coolant conductivity and feedwater copper concentration. Investigation determined the abscence of any major condenser tube leaks and a program to improve condensate filter domineralizer offectiveness was initiated. Reactor power was returrved to 100% on April 29 when feedwater chemistry returned to acceptable limits. Limerick Unit 2 end'.,d the month of April at a nominal 100% of rated thermal power.

Operational events that occurred during the month of April included:

The primary containment instrument Oas (PCIG) system suction and the PCIG '2B' header supply isolated during Installation of

  • banana jacks
  • In the system control circuitry These test jacks were being installed to accomodate PCIG emergency vent procedure testing activilles. During the test Jack installation, circuit continuity was broken, causing an isolation signa' to ' seal-in', Jumpers had been previously Installed to prevent closure of the outboard PCIG valves during test jack installation. Following test jack Installation, the jumpers were removed prior to resetting the isolation signal ' seal in' This caused the outboard isolation valves I to isolate the system. Back up was provided to the PCIG '2A' header by the Instrument Air system until the Isolation was reset and the PCIG system was returned to normal, ll. CHALLENGES TO MAIN STEAM SAFETY RELIEF VALVES There were no challenges to the Main Steam Safety Relief Valvesduring the month of April.
