REQUALIFICATION PROGRAM EVALUATION

During the week of March 12, 1990, the NRC conducted an evaluation
of the requalification program in place at the Maine Yankee Atomic
Power Plant by administering examinations to employees of your
company who operate the Maine Yankee plant.
,The evaluation was
conducted in accordance with NUREG-1021, Operator Licensine
Examiner Standards, ES-601, " Administration of NRC Requalification
Program Evaluations."
The evaluation had two purposes:
(1) to
evaluate the effectiveness of the licensed operator requalification
training program'in maintaining the competency of the licensed
operators, and (2) to examine the individual operators to determine
if they would satisfy the regulatory requirement of passing an NRC
administered examination prior to license renewal.
At the
conclusion of the evaluation, generic findings that evolved were

discussed with those members of your staff identified in the
enclosed report.
As evaluated by the NRC, both crews passed the simulator portion of
the examination and six of the seven operators passed all portions
of the examination.
One senior reactor operator failed the
simulator portion of.the examination.
Because less than twelve
operators were examined during this cycle, a programmatic
evaluation will be deferred until inclusion of the next cycle of
NRC administered requalification examinations.
The-senior reactor
operator who f ailed the examination was immediately removed from
licensed duties and assigned to a remedial training program
designed to address the individual weaknesses identified.
Upon
completion of the remedial training you are expected to
readminister a complete simulator examination.
After the
individual has been successfully remediated and reexamined he may
be returned to licensed duties.
During the evaluation, it was determined that one of the activities
inspected was an apparent violation of NRC requirements and your
operating license. _ (see paragraph 4 of the enclosed inspection
report.)
This violation is set forth in the Notice of Violation
enclosed as Appendix A of this letter.
The violation has been
categorized by severity level in accordance with the " General
Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR 2,
Appendix C (Enforcement Policy).
You are required to
respond to this letter following the instructions in Appendix A.

It is requested that you provide details as to how many personnel
have been exempted from the required examinations since the
implementation of the revised 10 CFR 55 in May, 1987 and what their
duties were at the time of exemption.
Additionally, corrective actions related to the open items
identified during the Emergency Operating Procedures inspection
(50-309/89-81) were reviewed.
Four of the open items were closed,
as identified in paragraph 8 of the enclosed inspection report.
However, your response to several of the open items included
completion dates that, the NRC considers, result in prolonged
deficiency resolution that is inconsistent with the significance of
the identified items.
You are requested to review this matter and
provide a supplemental response which has addressed the apparent
need for more timely deficiency resolution milestones.
In accordance with 10 CFR 2.790 of the Commission's regulations, a
,
copy of this letter and the enclosure will be placed in the NRC
Public Document Room.
