Appendix i-NOTICE OF VIOLATION Toledo Edison Company Docket No. 50-346/82-04 As a result of the inspection conducted on January 18-22, 1982, and in ac-cordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),
tl.e following violation was identified:
10 CFR 55, Appendix A states in part, "The requalification program shall include on-the-job training so that:...Each licensed operator and senior operator is cognizaat of facility design changes, procedure changes, and facility license changes."
Davis-Besse Procedure AD 1828.15.2, Requalification, Revision 2, states, "Each licensed individual will maintain an up-to-date knowledge of facility design changes, procedure changes, and facility license changes. Any of the following methods may be used.. 1. Required Reading Distribution (self study)..."
Contrary to the above, seven licensed operators and senior operators failed to complete individually assigned required reading, and therefore were not cognizant of design, procedure and facility license changes. This occurred for assignments covering the period from July to November 1981.
Six individuals had one or two assignments and one person had five assignments which were not completed during the period.
This is a Severity Level VI violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Consideration may be given to extending your response time for good cauce shown.
