Corrective Action Taken.
A light and radiation congruence check was performed in Fetruary 1982. The match was within + Imm. The film is on file.
Corrective Action Taken to Avoid Further Ncn-Compliance.
The light and radiation congruence check has been added to the monthly management check list and will be done monthly.
Date When Full Compliance Will be Achieved.
Full compliance has been achieved as of February 17, 1982.

CALIBRATION OF SIEMENS GAMMATRON S COBALT-60 MACHINE The following tests were performed on the Siemens Co-60 machine for compliance with the pertinent sections of 10 CFR 35.
35.21,b.1.
The exposure rate or dose rate to an accuracy within 1 3 percent for the range of field sizes and for the range of distances (or for the axis distance) used in radiation therapy.
On Febguary 15,198g the dose rate was determined for field sizes from 5x5 cm to 24x24 cm for 80 cm SSD treatment without the Lucite tray in position. The Victoreen Condenser-R-meter with the 100 R chamber serial 581 calibrated by the National Bureau of Standards in April of 1980 was used. The CF (correction factor) for it is 0.99. The results are as follows:

A film was exposed.by the radiation field.- The light field size was indicated by pin pricks on this film. There was a good match between the two. The films are on file.

35.21.b,5, The accuracy of all distance measuring devices used for treating humans shall be tested.
On February 17, 1982 the following checks were performed:
1.
Distance Indicators The light distance indicator was compared with the fixed mechanical distance indicator attached to the collimator over the distance range of 65 cm to 100 cm SSD at 5 cm intervals.
Agreement was found to be within j;2 mm at all distances..
2.
The laser distance indicators (80 cm SSD) were checked against the mechanical distance indicator and found to be accurate to within 3;2 mm.
3 The laser indicators were also checked for co-alignment with one another.
4.
The field size indicator was compared with the size of the light field. Agreement was found to be within agreement f; 2 mm.
