APPENDIX A NOTICE OF VIOLATION Public' Service Electric and Gas Company Docket No. 50-354 Hope Creek, Unit 1 License No. CPPR-120 As a result of the inspection conducted on January 4-31, 1982, and in accordance with the Interim Enforcement Policy, 45FR66754 (October 7,1980), the following violation was identified:
10 CFR 50 Appendix B, Criterion XII requires measures to control measuring and test equipment used in quality related activities. The PSAR Section 16.2.12 establishes procedures to control such equipment. The GEI&SE Nu-clear Quality Assurance Manual and implementing procedures. " Calibration of Measuring and Test Equipment," further require a log identifying the instruments and the specific inspection report or traveler and sequence where the device was used and specifies that equipment be returned to the control point at the end of each shift.
Contrary to the above, as of January 14, 1982, instruments used in quality activities were not properly controlled in that the log was not being main-tained nor were instruments being returned to the control point at the end of each shift.
This is a Severity Level V Violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to extending your response timp.
