SCOPE:
This inspectior, was made for two principal purposes:
(1) to assess the effectiveness of Ebasco's system for design verfication by in-depth' design reviews of specific safety-related systems on two active projects, Shearon Harris and Washington Nuclear Plants (WNP) 3 and 5; and (2) to verify com-pliance to design requirements of parallel slide gate valves procured by Ebasco from Atwood Morrill and fabricated by Hopkinsons Ltd. for use at WNP 3 and 5.

Design Inspection (Shearon Harris Unit 1) - The Ebasco Topical Report ETR-1001 and applicable procedures were reviewed to establish quality assurance program commitments.
Section 6.2.2 (Containment Heat Removal System) and 6.5.2 (Containment Spray System) of the Shearon Harris PSAR were reviewed and selected design inputs were picked.
The selected design inputs were then traced through the design process to the final output documents to verify that the design process had been correctly implemented.
Concurrent witn this verification, the effectiveness of Ebasco's design verification methods was assessed.
The following documents were examined to assess the effectiveness of the Ebasco design verification process:
2 design request memos, 2 calcu-lations, 1 electrical design criteria document, 4 specifications and 26 drawings.
In the documents examined, no nonconforma,ces or unresolved items were identified.
However, three items of concern were identified and will be further reviewed during future inspections.
First, the amount of docu-mentation retained by the design verifier makes it difficult, in some cases, to determine the methodology used by the design verifier.
Second, the method of design verification is not uniform throughout the design disciplines; e.g., the electrical disciplir.e does not permit the designer draftsman to perform design verification while other design disciplines do.
Third, design verification, in some disciplines, is done by supervisors, which can raise questions about true independence.

Design Inspection (WNP 3/5 Project) - Sections of the Washington Public Power Suppiy System (WPPSS) Unit 3 PSAR and draft FSAR pertaining to the containment spray system (CSS), and applicable design control procedures were reviewed to determine the commitments and requirements for the CSS and to check if design inputs of the CSS were identified, appropriate,

Selected CSS intermediate and final design documents, such as calculations, specifications, and drawings, were examined in concert with an ad hoc multidisciplined group designated by Ebasco management to participate in a limited scope design review of the CSS, to determine if the final design was traceable to, and met, the approved design inputs and performance requirements.
The group addressed approximately 160 questions related to regulatory and ASME Code requirements, pre-and post-accident containment environmental conditions, functions and performance requirements of the CSS, and five major components of the CSS that were identified by the NRC inspector.
Within the areas examined, no significant design deficiencies nor failure to implement commitments were identified.
However, the NRC inspector noted that:

The interpretation'and extent of independent design verification does not appear to be consistent within the WNP-3/5 project in that the I&C group performs a documented independent design verification of each design using an I&C engineer, who was not the originator of the design, in addition to a design verification by the supervising engineer as permitted by Ebasco procedure E-76.
In contrast, the mechanical group apparently does not document their independent design verification at the engineering level.

The Ebasco procedure for design verification (E-76) permits the use of a supervisory engineer to act as the independent design verifier under certain conditions.
Inconsistencies with respect to the safety relief valve for the sodium hydroxide tank between early and later revisions of the CSS flow diagram (WPPSS-3240-G-1070) and the associated safety relief valve specification (WPPSS Speci-fication 3240-91), and the apparent lack of calculations or other documentation to support the currently specified relieving capacity of the valve, generates a concern that in some cases, the supervisor may not have sufficient time to conduct a thorough and objective verification of design adequacy.

Parallel Slide Gate Valve Design Compliance - The parallel slide, or double gate, valve is used in numerous applications on the WNP 3 & 5 units.
This area of the inspection was to verify compliance to design requirements for valve body overpressurization protection, seismic quali-fication, and material control.
A 12" valve for use in the contain-ment spray system was used as a typical example for the purposes of this inspection.

PAGE 4 of 4 The subject valve (Ebasco drawing no. 3240-16562) has three 3/4" Schedule 80 pipes, up to 12.5" in length, welded to the valve body.
These three pipes provide the following features:
(1) " Pressure Relief Connection"-
a pipe with welded plug near the bottom of the valve body to be used for periodic flushing of the valve body; (2) " Blowout Connection"-a pipe located between the Stellite faced backseat and stem packing for ease of removal of packing for maintenance; and (3) " Leak-off Connection"-
a pipe located between the double packing in the stem to monitor the packing integrity.
Valve body overpressurization protection is provided in the valve-closed position by a balance valve installed at the centerline of the valve with welded pipe communicating the valve body with the high pressure side of the valve inlet.
In the valve-open position the valve body communicates directly with the line pressure.
Seismic qualification of these type valves by analysis and prototype tests was required.
The seismic analysis by Hopkinsons (Design Report No. 34) was reviewed for the subject valve which included the three pipe connections and the balance valve.
The prototype seismic (vibra-tion) tests were performed on the subject valve by American Environ-mental Labs in accordance with Atwood Morrill specification 556-14215-00.
The test results had not been received by Ebasco, as yet, and according to Ebasco Expediting Report No. 32, tests of the remaining typical valves would not be completed until the Spring of 1982.

The remai1ing piping for the " Blowout" and " Leak-Off" connections will completed at the WNP site.
According to the applicant's letter, PFA-35-81-108, the " Blowout Connection" will be sealed with a welded plug.
Ebasco drawing no. 3240-G-1096 is a typical for leak off piping and applies to the subject " Leak-Off Connection." The design and analysis of this piping will be performed by Ebasco site personnel.
l Ebasco specification 3240-41J requires all stainless steel used in these valves to be solution annealed; however, Material Certs are not sent to Ebasco's New York office and, therefore, could not be reviewed during this inspection.
Welding procedure specification N6-61 for socket and end cap butt welds in stainless steel was reviewed for compliance with Section IX and applicable parts of Section III of the ASME Code.
No nonconformances were ider.tified in this area of the inspection.
The subject valve drawing from the vendor was not a legible reproducible; however, Ebasco had initiated action to obtain one from the vendor.
