Investigation of all violations of the Technical Specifications and shall prepare and forward a report covering evaluation and recommendations to prevent recurrence to the Vice President Nuclear Operations and to the Chairman of the Nuclear Review Board. f. Review of facility operations to detect potential safety hazards. 6-6

Report to and be advisory to the Station Superintendent on those areas of responsibility specified in Section 6.5.1.6 and 6.5.1.8. b. See 6.5.1.6.j above. c. Provide timely written notification, meeting minutes may be used for this purpose, to the Vice President Nuclear Operations and the Chairman of the Nuclear Review Board of disagreement between the PORC and the Station Superintendent however, the Station Superintendent shall have responsibility for resolution of such disagreements pursuant to 6.1.1 above. 6. 5.1. 8 The PORC shall maintain written minutes of each meeting and copies shall be provided to the Vice President Nuclear Operations and Chairman of the Nuclear Review Board.

w ADMINISTRATIVE CONTROLS RECORDS 6.5.2.10 Records of NRB activities shall be prepared, approved and distributed as indicated below: a. Minutes of each NRB meeting shall be prepared, approved and forwarded to the Senior Vice President-Nuclear Engineering and Operations within 14 days following each meeting. b. Reports of reviews encompassed by Section 6.5.2.7.e, f, g, and h above, shall be prepared, approved and forwarded to the Senior Vice President-Nuclear Engineering and Operations with 14 days following completion of the review. Meeting minutes may be used for this purpose. c. Audit reports encompassed by Section 6.5.2.8 above, shall be forwarded to the Senior Vice President-Nuclear Engineering and Operations and to the management positions responsible for the areas audited within 30 days after completion of the audit. Meeting minutes may be used for this purpose. QUALIFICATIONS 6.5.2.11 Minimum qualifications of NRB members are as follows: a. Academic degree in an engineering or physical science field or equivalent as per Section 6.5.2.11.c. b. Minimum of five years technical experience, of which a minimum of three years is in one or more of the areas specified in Section 6.5.2.1. c. Technical experience in the areas specified in Section 6.5.2.1 beyond the three year minimum may be substituted on a one for one basis towards the academic degree requirement. (Four years of technical experience being equivalent to a four year academic degree). d. Total academic and technical experience must total a minimum of nine years. 6-12

ADMINISTRATIVE CONTROLS 6.6 REPORTABLE OCCURRENCE ACTION 6.6.1 The following actions shall be taken for all events which are required by regulations of Technical Specifications to be reported to the NRC in writing within 24 hours. a. The Commission shall be notified and/or a report submitted pursuant to the requirements of Specification 6.9. b. Each Reportable Occurrence Report submitted to the Commission shall be reviewed by the PORC and submitted to the NRB and the Vice President Nuclear Operations. 6.7 SAFETY LIMIT VIOLATION 4 6.7.1 The following actions shall be taken in the event a Safety Limit is violated. a. The provisions of 10 CFR 50.36 (c) (1) (i) shall be complied with immediately. b. The safety Limit violation shall be reported to the Commission, the Vice President Nuclear Operations and to the NRB within one hour. c. A Safety Limit Violation Report shall be prepared. The report shall be reviewed by the PORC. This report shall describe (1) applicable circumstances preceding the violation, (2) effects of the violation upon facility components, systems or structures, and (3) corrective action taken to prevent recurrence. d. The Safety Ilmit Violation Report shall be submitted to the Commission, the NRB and the Vice President-Nuclear Operations within 14 days of the violation. 6.8 PROCEDURES 6.8.1 Written procedures and administrative policies shall be established, implemented and maintained that meet or exceed the requirements and recommendations of Section 5.1 and 5.3 of ANSI N18.7-1976 and Appendix "A" of USAEC Regulatory Guide 1.33 except as provided in 6.8.2 and 6.8.3 below. Procedures shall be established and maintained for implementation of the Facility Fire Protection Program. 6.8.2 Each procedure and administrative policy of 6.8.1 above, and changes thereto, shall be reviewed by the PORC and approved 2 by the Station Superintendent prior to implementation and periodically as set forth in each document. 6-13

Millstone Unit 1 6.0 AIPd.INISTRATIVE CONTROLS 6.1 RESPONSIBILITY 6.1.1 The Station Superintendent shall be responsible for overall operation of the Millstone Station Site while the Unit Superintendent shall be respon-sible for operation of the unit. The Station Superintendent and Unit Superin-tendent -shall each' delegate in writing the succession to these responsibilities during their absence. 6.2 ORGANIZATION OFFSITE 6.2.1 The offsite organization for facility management and technical support shall be as shown in Figure 6.2-1. FACILITY STAFF 6.2.2 The Facility organi2ation shall be as shown on Figure-6.2-2 and: ~ a.. Each on duty shift shall be composed of at least the minimum shift crew composition shown in Table 6.2-1. ~ b. At least one licensed Operator shall be in the control roce when fuel is in the reactor. c. At least two licensed Operators shall be present f a the'. ~ control room during reactor start-up, scheduled reactor shutdown and during recovery from reactor trips. d. An individual qualified in radiation protection procedures shall be on site when fuel is in the reactor. e. The safety evaluation for 1) changes to procedures, equipment or systems and 2) tests or experiments completed under the provision of Section 50.59, 10 CFR, to verify that such actions did not constituta an unreviewed safety question. b. Proposed changes to procedures, equipment or systems which involve an unreviewed safety question as defined in Section 50.59,10 CFR. c. Proposed tests or experiments which involve an unreviewed safety question as defined in Section 50.59,10 CFR. d. Proposed changes in Sec'tions 1.0 - 5.0 of these Technical Specifi-cations or licenses. e. Violations of applicable statutes, codes, regul tions, orders. Technical Specifications, license requirements, or of internal procedures or instructions having nuclear safety significance, f. Significant operating abnormalities or deviations from normal and expected performance of plant equipment that affect nucienr safety. g. REPORTABLE OCCURRENCE, as defined in sections 6.9.1.7, 6.9.1.8 ~ and 6.9.1.9. l h. Indications of a significant unanticipated deficiency, affecting nuclear safety, in some aspect of design or operation of safety related structures, systems or components. i. Reports and meetings minutes of the PORC. AUDITS 6.5.3.8 Audits of facility activities shall be performed under the cognizance of the NRB, These audits shall encompass: a. Security Plan implementation. e. Emergency Plan implementation. f. I ADMINISTRATIVE CONTROLS 6.9.1.3 Startup reports shall be submitted within (1) 90 days following completion of the startup test program (2) 90 days following resumption or cor encenent of comercial power operation, or (3) 9 months following initial criticality, whichever is earliest. If the Startup Report does ,not cover all three events (i.e., initial criticality, completion of I startup test program, and resumption or commencement of commercial power operation), supplementary reports shall be submitted at least every three ronths until all three events have been completed. > ANW',L p.E PORTSO 6.9.1.4 Annual reports covering the activities of the unit as described below for the previous calendar year shall be submitted prior to March 1

( each year. The initial report shall be submitted prior to March 1 of the year following initial criticality.

6.9.1.5 A tabulation on an annual basis of the number of station, utility and other personnel (including contractors) receiving exposures greater than 100 mrem /yr an and job functions, -{their associated man-rem exposure according to work e.g., reactor operations and surveillance, inservice inspection, routine maintenance, special maintenance (described mainten-ance), vaste proce4; sing and refueling. The dose assignment to various duty functions may be estimates based on pocket dosimeter, TLD or film badge measurements. Small exposures totalling less than 20% of the individual total dose need not be accounted for. In the aggregate, at least 80% of the total whole body dose received from external sources shall be assigned to specific major work functions. ~1/ A s.ngle submittal may be made for a multiple unit station. ADMINISTRATIVE CONTROLS MEETING FREQUENCY 6.5.1.4 The PORC shall meet at least once per calendar month and as convened by the POP.C Chairman. QUORUM 6.5.1.5 A quorum of the PORC shall consist of the Chairman or Vice Chairman or Station Superintendent and four members including alternates. RESPONSIBILITIES 6.5.1.6 The PORC shall be responsible for: Review of 1) all procedures, except common site procedures, e. required by Specification 6.8 and changes thereto, 2) any other proposed procedures or changes thereto as determined by the Unit Superintendent to affect nuclear safety. b. Review of all proposed tests and experiments that affect ' nuclear safety. Review of all proposed changes to Sections 1.0 - 5.0 of these c. Technical Specifications. d. Review of all proposed changes or modifications to plant systems or equipment that affect nuclear safety. Investigation of all violations of the Technical Specifications e. and preparation and forwarding of a report covering evaluation and recommendations to prevent recurrence to the vice President Nuclear Operations and to the Chairman of the Nuclear Review Board. f. Provide immediate written notification to the Station Superin-tendent Vice President Nuclear Operations and the Chairman of the Nuclear Review Board of disagreement between the PORC and the Unit Superintendent; however, the Unit Superintendent shall have responsibility for resolution of such disagreements pursuant to 6.1.1 above. RECORDS 6.5.1.8 The PORC shall maintain written minutes of each meeting and copies shall be provided to the Station Superintendent, Vice President Nuclear Operations and Chairman of the Nuclear Review Board. 6.5.2 SITE OPERATIONS REVIEW COMMITTEE (SORC) FUNCTION 6.5.2.1 The 50RC shall function to advise the Station Superintendent on all matters related to nuclear safety of the entire Millstone Station Site. AUTHORITY 6.5.2.'7 'The SORC shall: a. Recommend to the Station Superintendent written approval or disapproval in meeting minutes of items considered under 6.5.2.6(a) through (g) above. b. Provide immediate written notification or meeting minutes to the Vice President Nuclear Operations and the Chairman of the Site Nuclear Review Board of disagreement between the 50RC and the Station Superintendent; however, the Station Superintendent shall have responsibility for resolution of such disagreements pursuant to 6.1.1 above. RECORDS 6.5.2.8 The SORC.shall maintain written minutes of each meeting and copies shall.be provided to the Vice President Nuclear Operations and Chairman of the Site Nuclear Review Board. 6.5.3 NUCLEAR REVIEW BOARD (NRB) FUNCTION 6.5.3.1. The NRB shall function to provide independent review and audit of designated activities in the areas of: a. nuclear power plant operations b. nuclear enginee' ring c. chemistry and radiochemistry d. metallurgy e. instrumentation and control f. radiological safety g, mechanical and electrical engineering HILLSTONE - UNIT 2 6-9

l ADMINISTRATIVE CONTROLS AUTHORITY 6.5.4.9 The SNRB report to and advise the Senior Vice President Nuclear Engineering and Operations on those areas of responsibility specified i in Sections 6.5.4.7 and 6.5.4.8. RECORDS 6.5.4.10 Records of SNR8 activities shall be prepared, approved, and distributed as indicated below: Minutes of each SNRB meeting shall be prepared, approved, and a. forwarded to the Senior Vice President Nuclear Engineering and Operations within 14 days following each meeting. b. Reports of reviews encompassed by Section 6.5.4.7 above shall be prepared, approved, and forwarded to the Senior Vice President Nuclear Engineering and Operations within 14 days following completion of the review. Audit reports encompassed by Section 6.5.4.8 above shall be c. forwarded to the Senior Vice President Nuclear Engineering and Operations and to the management posi.tions responsible for the areas audited within 30 days after completion of the audit. 6.6 REPORTABLE OCCURRENCE ACTION 6.6.1 The following actions shall be taken for REPORTABLE OCCURRENCES: The Commission shall be notified and/or a report submitted a. pursuant to the requirements of Specification 6.9. b. Each REPORTABLE OCCURRENCE Report requiring 24-hour notifica-tion to the Commission shall be reviewed by the PORC and submitted to the NRB and the Vice President Nuclear Operations. 6.7 SAFETY LIMIT VIOLATION 6.7.1 The following actions shall be taken in the event a Safety Limit is violated: The provisions of 10 CFR 50.36(c)(1)(i) shall be complied with a. immediately. ADMINISTRATIVE CONTROLS SAFETY LIMIT VIOLATION (Continued) b. The Safety Limit violation shall be reported to the Comission, the Vice President Nuclear Operations and to the NRB immediately. c. A Sa.fety Limit Violation Report shall be prepared. The report shall be reviewed by the PORC. This report shall describe (1) applicable circumstances preceding the violation, (2) effects of the violation upon facility components, systems or structures, and (3) corrective action taken to prevent recurrence. d. The Safety Limit Violation Report shall be submitted to the Comission, the NRB and the Vice President Nuclear l Operations within 10 days of the violations. 6.8 PROCEDURES 6.8.1 Written procedures shall be established, implemented and maintained covering the activities referenced below: a. The applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33, November 1972. b. Refueling operations, c. Surveillance activities of safety related equipnient, d, Security Plan implementation, e. Emergency Plan implementation. f. Fire Protection Program implementation. 6.8.2 Each procedure and administrative policy of 6.8.1 above, and changes thereto, shall be reviewed by the PORC/SORC, as applicable, and approved by the Unit Superintendent / Station Superintendent prior to implementation and reviewed periodically as set forth in each document.
