Appendix NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-373 Docket No. 50-374 As a result of the inspection conducted on December 9, 10, 14, 23, 29, 30, 1981, and January 7, 11, and 15, 1982, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criteria VII requires that measures shall be estab-lished to assure that purchased material, equipment, and services, whether/
purchased directly or through contractors and subcontractors, conform to the procurement documents. Documentary evidence that material and equipment conform to the procurement requirements shall be available at the nuclear power plant... site prior to installation or use of such material and equipment.
The Commonwealth Edison Company Quality Assurance Manual Quality Procedure No. 7-1, states that: Equipment, components, or materials in an undamaged condition which lack the required quality documentation may be given interim release on the CECO Material Receiving Report by Site QA for installation (Not on " hold") provided that for safety-related, non-ASME Section III code items, an acceptable certificate of conformance is received.
" Items in a QA hold status may be installed prior to resolution of the hold in accordance with QP No. 15-1 of the Commonwealth Edison QA Manual."
QP No. 15-1 states in part, "Upon detection by Commonwealth Edison Company of an on-site nonconformance, a nonconformance report (NCR), Form 15-1.1,.is initiated. Nonconformance items discovered on-site shall be identified and controlled by a " hold tag."
Contrary to the above, MRR No. 6125 documents the receipt on February 2, 1979 of reels of instrument cable, Specification J2967, P.O. No. 181714, including reel No. B0159 part of which was installed in a safety-related system, cable IVY 100, on March 26, 1979, without having received the required Cirtificate 8202250166 820211 PDR ADOCK 05000373 G
Appendix of Conformance, without the required Interim Release Status, without the required hold tag and hold status, and without a Nonconformance Report having been written.
This is a Severity Level V Violation (Supplement II).
The inspection showed that action had been taken to correct the identified item of noncompliance and to prevent recurrence.
Consequently, no reply to this item of noncompliance is required and we have no further questions regarding this matter.
