I APPENDIX A NOTICE OF VIOLATION Pubite Service Electric and Gas Company Docket No. 50-354 Hope Creek, Unit 1 License No. CPPR-120 As a result of the inspection conducted on December 1,1981 - January 3,1982, and in accordance with the Interim Enforcement Policy, 45FR66754 (October 7,1980), the following violations were identified:
A.
10 CFR 50 Appendix B, Criterion XVI requires that measures be established to promptly identify and correct conditions adverse to quality.
Section 16.2.16 of the Hope Creek Generating Station Unit 1 PSAR states that provisions exist for the identification of significant discrepancies and that corrective actions are required to detemine effectiveness and the need for future corrective action.
Contrary to the above, corrective actions for violations of weld filler mater-tal issue procedures in September,1980, were ineffective in that a review of recent weld records found welds for which the procedures were not implemented.

10 CFR 50 Appendix B Criterion V requires that activities affecting quality be accomplished in accordance with instructions, procedures, or drawings.
Bechtel Drawing C-0973-0, Revision 5. " Primary Containment Drywell Interior Framing Typical Details and Schedules," states that in case of bolt hole mis-alignment, bolt holes may be slotted and friction type connections used pro-vided that all contact surfaces are blast cleaned from paint and other impur-ities, proper bolt tension is accomplished by load indicating washers, and only holes in the web of the rolled section are slotted.
Contrary to the above, as of September 1,1981, slotted holes were in general use, bolt holes were slotted in the angle clips rather than in the web of the rolled section, faying surfaces of bolted connections were painted, and load indicating washers were improperly installed.
This is a Severity Level V Violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201 Public Service Electric and Gas Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective stepswhichhavebeentakenandtheresultsachieved;(2)correctivestepswhich will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affimation. Where good cause is shown. consideration will be given to extending your response time.
