V. Crlenjak on May 5, 1981 in accordance with 10 CFR 50.55(e) as NCR's 1447 and 1457. This as followed by our interim reports dated June 5, August 19, and November 12, 1981. Enclosed is cur fourth interim report. We expect to submit our next report by. April 15, 1982. TVA.

FOURTH INTERIM REPORT Description of Deficiency INRYCO, Incorporated, is the contractor, contract No. 75C53-85380, for supplying and installing the prestressing system for the primary containments at Bellefonte Nuclear Plant units 1 and 2.

On April 15, 1981, INRYC0 informed TVA that a shophead/ bushing failure had occurred during stressing operations on March 19, 1981 at the Braidwood Nuclear Plant and that INRYC0 had submitted a report in accordance with 10 CFR Part 21 to the Nuclear Regulatory Commission to document this failure. As a result of this failure, INRYC0 began measuring the unstressed anchorage material at Bellefonte. Several cases have teen documented whereby the dimensions measured do not fall within the tolerances established by the shop fabrication manual.

In some instances, the anchorage material is cversized and in other instances, the anchorage material is undersized.

Interim Progress The third interim report on this deficiency stated that Inryco had submitted their final report. TVA has evaluated Inryco's report and provided comments to Inryco.

Inryco has resubmitted the final report and TVA is now awaiting Inryco's submittal of all anchorage measurements before finalizing a disposition of this deficiency.

All NCR's concerning dimensionally nonconforming anchorage material will be handled under this report (BLRD-50-438/81-33, -439/81-36). To date these include 1447, 1457, 1461, 1469, 1473, ~ 1474, 1475, 1485, 1491, 1502, 1510, 1511, 1521, 1523, 1542, 1547, 1558, 1562, 1571, 1590, 1591, 1628, 1629, 1630, 1642, 1643, 1650, 1657, 1670, 1678, 1698, 1704, and 1712.
