Appendix NOTICE OF VIOLATION Illinois Power Company Docket No. 50-461 As a result. of the inspection conducted on December 1-31, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion V states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instruc-tions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Clinten Power Station Final Safety Analysis Report (FSAR), Paragraph 17.2.5 states: "Each Company organization is responsible for developing, reviewing, approving, issuing, and using formal instructions, procedures, drawings, or related material for performing activities affecting the quality or functions of applicable systems, structures, or components at CPS."
Baldwin Associates Quality Assurance Manual, Paragraphs 5.4.1 and 5.4.2 state:
" Written work instructions, procedures, and drawings prepared by Baldwin Associates shall contain adequate acceptance criteria to assure that safety-related activities have been satisfactorily accomplished.
Activities affecting quality shall be controlled by documented instructions, procedures, or drawings."
Contrary to the above, the Contractor was performing cleaning activities on the Steam Separator (Reactor Internal) without the use of written procedures or instructions.
This is a Severity Level V violation (Supplement II).
Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written state-ment or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full com-pliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Consideration may be given to extending your response time for good cause shown.
