AFFIDAVIT OF C. H. GATCHELL REGARDING CONTENTION SEVEN (ROCK OVERBREAK) 1 Charles H. Gatchell, being first duly sworn, do depose 2
and state:
I was employed by Texas Utilities Services, Inc. in the position of Resident Manager from July 16, 1973 to June 16, 1977.
During that time, I was responsible for assuring that all concrete p1acements for seismic Category I structure foundations were done in accordance with the required specifications and procedures. My educational and professional qualifications are attached to this Affidavit.
This Affidavit addresses whether, during my tenure as Resident manager, any loose rock and other foreign material were thrown into the excavation site prior to or during the p1acement of concrete.
It will also address whether any corrective action had to be taken as a result of loose rock, foreign materials and voids.

4 Based on my personal observation and my examination of all of the applicable pour cards, nonconformance reports and other documents as appropriate, I can attest to the fact that there were no instances in which loose rocks or other foreign materials were thrown into the site before concrete placements began for foundations of seismic Category I structures.
Further, with the exception of the four incidents below, I can attest to the fact that all concrete placements (including those necessary to repair incidents of rock-overbreak) were done in accordance with all applicable specifications and procedures.
There were two instances in which during or after concrete pours foreign material was found in seismic Category I foundations.
First, a chainfall was imbedded in the Unit I base mat. The increased mat stresses resulting from the chainfall were found to be negligible and for that reason no repairs were required.
In addition, two wood spreaders used to maintain form to steel clearance were left 17 a placement at the 797' elevation.
The spreaders were removed by drilling and chipping and the cavity repaired.
Two incidents of voiding were also reported. Visual inspection indicated the presence of voids under both sumps in the 4
Unit I containment base mat. The average void space was 3/8" with a maximum void of 5/8".
To repair these voids, they were filled with a neat cement grout placed under low pressure.
In addition, concrete was not properly worked around one of the valve embedments.
As a result, a void was evident under the embedment. The void was filled in with concrete.
Based on (Report on Unit 1 Containment Mat Evaluation, G8H, June 18, 1976), I concluded that there are no foreign material, loose rock and/or voids in the foundations of seismic Category I structures which will have an effect on the static, dynamic and engineering proporties of those structures.
