APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electric Company Dockets 50-317, 50-318 Calvert Cliffs Units 1 and 2 Licenses DPR-53, DPR-69 During the inspection conducted on December 1,1981 - January 5,1952 and in accor-dance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), i.he follow-

, ing violations were identified.

A.1 Technical Specification 6.8.1 requires establishment of and adherence to the written pmcedures recomended in Appendix A of Regulatory Guide 1.33, Novem-ber 1972, which specifies that procedums for radioactivity control include personnel monitoring, restrictions and activities in radiation areas, surveys and monitoring, and radiation work pemit procedures. Calvert Cliffs Instruc-tion 4008 requires anyone perfoming work under a Radiation Work Pemit to read and understand instructions of the pemit before entering a radiation area, to certify this action by initialling the posted Radiation Work Pemit, and to monitor themselves for contamination at the Access Control Point upon exiting the radiation controlled area.

Contrary to the above, on December 24,1981, a technician entered the Unit 1 Refueling Water Tank Room (a radiation controlled area) without initialling the Radiation Work Pemit and exited that area without monitoring himself for contamination.

This is a Severity Level V Violation (Supplement IV) applicable to DPR-53.

A.2 Technical Specification 6.8.1 requires establishment of and adherence to the written procedures recomended in Appendix A of Regulatory Guide 1.33, Novem-ber 1972. which specifies proceduns for.~ervice Water Systems. Calvert Cliffs Operating Instruction 15, Service Water System, Revision ll, approved December 16,1980, requires that certain valves be locked in appropriate positions.

Contrary to the above, on December 21,1981, the following valves were found i

to be in the correct positions but wem not locked as required.

A.3 10 CFR 50 Appendix B Criterion V req 6 ires that activities affecting quality is prescribed and accomplished in accorde7e with appropriate instructions, pro.s-dures, and drawings. Qur.lity Assurance Program for Operations Section IC.5, April 30,1975, requires that methods and acceptance criteria for complying with applicable criteria be specified in instructions, procedures, and drawings. boric acid solution is piped in heat traced and insulated lines to preclude boric acid precipitation. Drawings 6750-E-39-7-12. Electric Heat Tracing Unit 110.1 Boric Acid System. Revision 13 and 6750-E-39-36-3. Electric Ileat Tracing Unit flo. 2 Boric Acid System, Revision 2, require that pipes be insu-lated with calcium silicate insulation, Contrary to the above, the following pipe sections were not insulated on the dates indicated:

December 11,1981 The suction elbow in the supply to Boric Acid Pump 21.

The suction line from Boric Acid Tank 21 to the first tee.

A section about 12 inches long in the Boric Acid Pump 22 minimum flow line.

b.

December 30, 1981 About 10 feet of Boric Acid Gravity Feed Piping to the Unit 2 CHV Pumps.

This is a Severity 1.evel V Violation (Supplement I) applicable to DPR-69.

Pursuant to 10 CFR 2.201, Baltimore Gas and Electric Company is hereby required to submit to tizis office within thirty days of the date of this Notice, a written reply stating the corrective ste;n which have been taken and the results achieved, correc-tive steps to be taken to avoid further violations, and the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under cath or affirmation.
