SCOPE:

This inspection was made as a result of: (1) the discovery of reactor coolant pump closure stud wastage due to boric acid corrosion at Oconee, Units 2 and 3; (2) the identification of a missing keyway on the shaft of a reactor coolant pump at Bellefonte, Unit 2; (3) a 10 CFR Part 50.55(e) construction deficiency report pertaining to improper fit between the impeller and shaft of a reactor coolant pump at Bellefonte, Unit 1; (4) a 10 CFR Part 21 report con-cerning the furnishing of an emergency feedwater pump lube oil cooler to Virgil C. Summer, Unit I which did not meet the design requirements of the ASME Code; and (5) the misorientation of the lubricant oil cooler on the high pressure l

NONCONFORMANCES:

Contrary to Criterion V of Appendix B to 10 CFR Part 50 and Section 3 of the OA Manual, the materials list for Sales Order No. 14210471 (South Carolina Electric and Gas Company-Virgil C. Summer, Unit 1) was not prepared from the Design Specification and Code in regard to imposition of required ASME Section III Class 3 Code for the Emergency Feedwater Pump turbine lube oil cooler.

Contrary to Criterion V of Appendix B to 10 CFR Part 50 and Bingham-Willamette Company (BWC) Procedure A14.0, a determination was not made, in regard to identified misorientation of lube oil cooler heads on high pres-sure make-up injection pumps at WPPSS-WNP 1/4, of whether similar defi-ciencies existed in equipment supplied to other locations.

OTHER FINDINGS OR COMMENTS:

1.

Follow-up Inspection on Reactor Coolant Pump Closure Studs Wastage due to Boric Acid Corrosion It was discovered that four closure studs on a reactor coolant pump (RCP) 2A2 at Oconee, Unit 2, had apparent corrosion wastage as a result of a small amount of leakage from the pump closure gaskets.

The pump was manufactured by BWC, and as a result of this deficiency, Babcock & Wilcox in conjunction with BWC developed a RCP gasket improvement over the type 316 stainless steel that had been used on the RCP 2A2.

This deficiency is not isolated to BWC in that IE Information Notice No. 80-27 identified this deficiency on pumps fabricated by other manufacturers.

Follow-up Inspection on the_ Lack of a Keyway on the Shaft of Reactor Coolant Pump 2PlAl As the result of a review of the documentation record packages for eight reactor coolant pumps supplied to TVA by BWC, it was discovered that some of the pump shafts may lack a floating throttle keyway.

Drawing No. Z7757, Revision L of June 4,1975, removed the keyway to accomodate a new type of seal for test and Revision N of May 3,1976, added the keyway back to the shaf t.

Consequently, shafts that were machined to Drawing No. Z7757, Revision L or M, would not have the required keyway.

A review of records revealed that only one shaft had been machined to Revision L or M and that was shaf t SN4 HT No. 535771-3 for reactor coolant pump 2 PIA 1.

The shaft was returned to BWC and the keyway machined on September 30, 1981.

10 CFR Part 50.55(e) Construction Deficiency Report A reactor coolant pump, IPA 2, manufactured by BWC and supplied to TVA, Belle-fonte, Unit 1, had an improper fit between the shaft and the impeller. A post test inspection report of August 26, 1975, required additional clearance due to expansion and the taper of the shaft was reground and inspected as required by Engineering Rider No. 112.

The surface contact of the impeller to shaf t was checked in accordance with Drawing No. D 15334 but Engineering failed to require a critical dimension of the spacer to be taken to assure seating of the impeller to shaft when the pump was assembled.

4 10 CFR Part 21 Report On June 6,1981, South Carolina Electric and Gas Company reported under the provisions of 10 CFR Part 21, a substantial safety hazard at Virgil C.

Summer Nuclear Station, Unit 1.

A turbine driven emergency feedwater pump lube oil cooler was not procured by BWC from their subvendor Terry Turbine Corp., to meet the design requirements of ASME Code,Section I!I, Class 3.

In a service letter of July 12, 1977, from Design to Contracts it was stated that the lube oil cooler had to be fabricated to the requirements of the ASME Code,Section III, Class 3.

Contracts responded by stating that the oil cooler will not be code-stamped.

As a result of this correspondence the inspector presented the following questions at the exit meeting:

a.

Since Design knew that the oil cooler required the quality provisions of ASME Section III, relative to temperature and pressure, why did they accept the decision of Contracts to down grade the item?

b.

What are the qualifications of Contract personnel to determine the quality requirements of safety-related equipment?

c.

What involvement, if any, did Quality Assurance have in this decision?

Management made no comment relative to these questions.

BWC has replaced the lube oil cooler with a heat exchanger which meets all the design and Code requirements; however, they have not reviewed documentation to assure that other turbines have not been purchased with similar design deficiencies as of the date of this inspection. (See nonconformance, item B.1).

Follow-up Inspection of Misorientation of the Heads on Lubricant Oil Coolers Babcock & Wilcox (B&W) notifed the NRC of a 10 CFR Part 21 report on January 9,1981, relative to the possible misorientation of the integral heads opposite the inlet flow line on lubricant oil coolers, thus causing reduction of flow through the coolers and an attendant reduct.on of heat trans fer.

These lubricant oil coolers are an integral part of the high pressure make-up injection pumps at WPPSS-WNP 1 and 4.

The subject pumps were fabricated by BWC; the lubricant oil coolers were manufactured by AMETEK, Inc.; and purchased by BWC,

PAGE a of a BWC informed of this safety-related deficiency in a letter from B&W on January 20, 1981, and was asked to review and make a recommendation for positive alteration to the cooler head to prevent misorientation of the head during assembly. BWC Purchasing responded on May 18, 1981 (MSG No. 1952) stating that B&W had taken adequate corrective action and that hardware modifications were not necessary.

In BWC inter-office correspondence of February 27, 1981, Field Engineering Service was requested by QA to determine if other units were affected in the field and take actions to correct and mark heads to avoid mislocation during re-assembly.

No actions had been taken by Field Engineering Service as of the date of this inspection. (See nonconformance, item B.2).
