This refers to the routine safety inspection conducted by Mr. J. Mattia of this office

on October 30 - November 30, 1981, at the Millstone Nuclear Power Station, Unit 3,
Waterford, Connecticut, of activities authorized by NRC License No. CPPR-113 and to
!
the discussions of our findings held by Mr. Mattia with Mr. S. Toth and others of
your staff at the conclusion of the inspection.
Areas examined during this inspection are described in the Office of Inspection and
Enforcement Inspection Report which is enclosed with this letter. Within these areas,
the inspection consisted of selective examinations of procedures and representative
records, interviews with personnel, and observations by the inspector.
Dased on the results of this inspectior., it appears that two of yGur activities were
not condv;ted in full compliance with HRC requirements, as set forth in the Notice
of Violation, enclosed herewith as Appendix A.
These items of noncompliance have
been categorized into the levels described in the Federal Register Notice (45 FR 66754) dated October 7, 1980. You are required to respond to this letter and in pre-

paring your response, you should follow the instructions in A pendix A.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosures will be

placed in the NRC Public Document Room unless you notify this office, by telephone,
.

within ten days of the date of this letter and submit written application to with-
hold information contained therein within thirty days of the date of this letter.
'
Such application must be consistent with the requirements of 2.790(b)(1).
