SCOPE: This inspection was made as a result of the issuance of a 10 CFR Part 21 Report pertaining to inadequate separation of Class IE and Non-Class IE conductors in the 480 volt load center isolation relay cabinets that have been furnished to the Palo Verde Nuclear Generating Staticn, Unit Nos.1, 2, and 3, An additional area of inspection was follow up on an unresolved item.

VIOLATION:

Contrary to paragraph 21.51(b) of 10 CFR Part 21, a record of evaluation had not been prepared for the identified Class IE/Non-Class IE conductor separation problem in the 480 volt load center isolation relay cabinets furnished to the Palo Verde Nuclear Generating Station, Unit Nos.1, 2, and 3.

NONCONFORMANCE:

Contrary to the requirements of Quality Assurance Procedure No.10.7, Revision 2, dated April 18, 1978, 100% inspection of crimped terminal lugs on nuclear switch-gear had not been performed, as evidenced by the lack of records to substantiate accomplishment of this activity.

STATUS OF PREVIOUS INSPECTION FINDINGS:

(Resolved) Unresolved Item (81-01):

It was not apparent to the NRC inspector that 100% inspection of crimped terminal lugs, on nuclear switchgear, had been performed.

The inspector verified that there were no records to substantiate that 100%

inspection of crimped terminal lugs on nuclear switchgear had been perfonned.

This item has therefore been closed and replaced with the nonconformance identified in paragraph B above.

OTHER FINDINGS OR COMMENTS.

1.

Follow up on a 10 CFR Part 21 Report - The engineering drawings related to the Isolation Relay Cabinet did not provide quantitative data relative to the separation criteria.

Rather, the information is depicted by symbols with statements that wires are to be run in front of or behind barriers.

It appears that insufficient data / instructions were a contributing factor to inadequate separation of Class IE/Non-Class IE conductors.

The inspector was informed that the hardware was arrected at the site by rerouting the wires; hcwever, no documents were provided to support this statement.

Drawings of current projects, with conductor separation requirements, are more specific, inasmuch as the requirement has been quantified.

A Gould-Brown Boveri letter, dated February 9,1981, to Bechtel Power Corporation states, in part, "The BBE evaluation of this condition indicates that this type of condition is not reportable to the NRC under the require-ments of 10 CFR Part 21."

One violation was identified as a result of there being no record of this evaluation.

Follow Up on an Unresolved Item - Terminal lugs are crimped onto conductors and connections made to devices or terminal strips. The crimped terminal lugs on nuclear switchgear are required to be 100% inspected; however, the crimped terminal lugs / conductors are not disconnected from the devices or

PAGE 3 of 3 terminal strips to facilitate inspection of the crimped terminal lugs.

Consequently, some required Inspection features are not visible for inspection; e.g., (1) wire range stamped on terminal; and (2) nicked or missing conductor strands, especially on conductors connected closest to the device or terminal strip of multi-terminal connections.
