5 9 Brown Boveri Electric, Incorporated Switchgear Systems Division Docket No. 99900740/81-02 Based on the results of an NRC inspection conducted on October 26, 27, and 30, 1981, it appears that certain of your activities were not conducted in accord-ance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions,
procedures, or drawings shall include appropriate quantitative.or qualitative acceptance criteria for determining that important activities have been satis-factorily accomplished."
Nonconformance with this requirement is as follows:
Quality Assurance Procedure No.10.7, Revision 2, dated April 18, 1978, requires: (1) 100% inspection of crimped terminals on nuclear switchgear; (2) inspection of the crimped terminals as part of the final switchgear checklist; and (3) use of AMP Publications 1S1261 and 1S1541.. Paragraph 3 of AMP Publication No. 151261 requires inspection of crimped terminals for
~the features identified in Figure 5.
This figure, in turn, lists seven accept and eight reject features. Among the accept features is: " Wire size is within wire range stamped on terminal or splice"; while an item among the reject features is: " Nicked or missing conductor-strands."
Contrary to the above,100% inspection of crimped terminal lugs on nuclear switchgear had not been performed.
Crimped terminal' lugs / conductors were not disconnected during performance of the final switchgear checklist. Conse-quently, the following features were not visible for inspection: (1) wire range stamped on terminal; and (2) nicked or missing strands. This was especially true of conductors connected closest to devices or terminal strips of multi-terminal connections.
