DESCRIPTION OF DEFICIENCY

Borg Warner 8" motor operated valve manufactured prior-to May 1979 was found, during Borg Warner gate guide testing, to experience '
failure during opening or closing if installed between 221/20 and 157 1/20 of vertical. One of these valves-(Tag.No.'MOV-1JS1BUV0672)
' supplied by Combustion Engineering as part of the NSSS is installed on a-horizontal plane in the safety injection system and therefore subject.to failure _as described by the attached Borg Warner Nuclear Valve Division letter. dated 10-20-81.
II.
ANALYSIS OF SAFETY IMPLICATIONS This condition.is evaluated as reportable since if not detected and corrected, the failure of this valve to open would prevent the Containment Spray System from operating.
III.
CORRECTIVE ACTION Borg Warner has coordinated the modification of this valve as described in the attached Borg Warner letter. ' Valves manufactured and shipped after May 1979 had design changes to-preclude recurrence of-this problem.

- and 157 of vertical. Damage to the body guide rail from the sharp edges on the gate ' guide can cause the valves to jam during opening or closing. Design changes to add a radius were accomplish-ed in January and May 1979 to preclude this situation.
Our records show that we have shipped similar valves to your project. (dentifies the resper.tive project and lists all the valves that have been shipped prior to January and May 1979 that are affected. In order to determine the extent of the corrective action, we need to know if any of the valves listed on the attachment will be installed between 22h and 157@
of vertical. Also of interest to us, is the system in which the listed valves are or will be installed. This information will assist us in deter-mining the necessity of additional notifications to the Nuclear Regulatory Commission.
Transmit all the information to us as soon as possible. If corrective action is necessary, we will rotifyyou ard ask your help to determine the most convenient manner to accomplish this modification.
