Docket No. 50-361 APPENDIX A Notice of Deviation Based upon 'the results of an NRC investigation conducted during the period of October 5 to 9,1981 it appears that one of your activities was not conducted in conformance with your commitments to the Comission

as indicated below:
A.
Contrary to~the licensee's commitment in paragraph 8.3.3.3.1 of the FSAR, which states, in part, that "... fire stops and breaks are furnished to provide a method of completely sealing off air spaces around cable penetrations..." -four fire stops were-found to allow significant air passage due to voids or incomplete fill of blocked-out areas. in.the penetrations. The penetrations involved are identifed as seal nos. AC-50-310F-3003, AC-50-310G-3007 AC-50-315-50148, and AC-50-315-5019A.
Each of these penetration seals 'had been inspected and accepted by the contractors quality control organization.
Based upon your actions to retrain cognizant personnel, repair deficient seals, and verify the integrity of all other seals no written reply to this Notice of Deviation is required. The effective-ness-of your corrective measures will be verified in a subsequent NRC inspection.
