APPENDIX A NOTICE OF VIOLATION Maine Yankee Atomic Power Company Docket No. 50-309 Maine Yankee Nuclear Power Station License No. DPR-36 As a result of the inspection conducted on November 12 - December 15. 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified:

Technical Specification 5.11 requires procedures for personnel radiation protection to be approved, maintained and adhered to for all operations involving personnel radiation exposure.

Radiation Protection Procedure 9-1-10, Revision 10. " Radiation Work Permits", requires the Working Party Leader to verify that each worker has read and signed the Radiation Work Permit (RWP) indicating that t e understands the provisions of the RWP before entering the work area; and requires any individual entering a high radiation area to be provided with the radiation monitoring device or other radiation control measure specified by the RWP.

RWP 81-11-64 covered work in the charging pump cubicles and required workers to carry a radiation survey meter.

Contrary to the above, during power operations on November 13, 1981, work party members entered the charging pump cubicles (posted "High Radiation Area: RWP Required") without signing the approved RWP (Serial #81-11-64) and without the radiation survey meter specified by the RWP.

This is a Severity Level V violation.

Pursuant to the provisions of 10 CFR 2.201, Maine Yankee Atomic Power Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:

(1)the correctivestepswhichhavebeentakenandtheresultsachieved;(2) corrective steps which will be taken to avoid further violations; (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is sho e, consideration will be given to extending your response time.
