Inspection Summary
Inspection on November 10-13, 1981 (Report No. 50-483/81-25)
Areas Inspected: Observation of electrical cable installation and termina-
tion activities; review of cable installation and termination records and
procedures; and review of related quality records. This inspection in-
volved a total of 30 inspection-hours on site by one NRC inspector.
Results: Of the areas inspected, one apparent item of noncompliance was
identified.

Licensee Action on Previously Reported Items
(Closed) Unresolved Item (50-483/80-15-01): Based on reports thit some
Rockbestos electrical cable had been spliced, the licensen was requested
to determine if splices might also exist in cable that had been purchap'd
and installed at Callaway.
Bechtel Power Corporation letter BLSE-8489, dated .'uly 10, 1980, states in
part, "Rockbestos has stated that no cable butt splices are made on the
SNUPPS orders.
Inspection of Rockbestos cable at the Woli Creek jobsite
and of sample reels at Rockbestos have found no evidence of butt splices."
(Closed) Open Item (50-483/81-20-03): During a previous inspection, minor
errors were noted in procedures WP-303 and QCP-303 that the licensee agreed
to correct. The errors in the subject procedures have been corrected.
(Closed) Unresolved Item (50-483/81-20-02): Daniel Procedures WP-303,
Revision 8, and QCP-303, Revision 7, did not address how the licensee
was going to verify that the cable pulling tension had not been exceeded
when small electric cables and/or instrument cables were pulled. This

item is upgraced to an item of noncompliance, see paragraph B.2 of this
report.

(Closed) 10 CFR 50.55(e) Report (50-483/79-03): Defective type SB-12
auxiliary switches on GE metalclad switchgear. The inspector reviewed
Union Electric's final report to Region III (ULNRC-322, dated August 10,
1979) with attached report and nonconformance report (NCR) number
2SN-0822-E which was prepared to track this item. Action to replace the
15 defective SB-12 switches has been completed and the subject NCR was
closed on March 12, 1981.
Functional or Program Areas Inspected
A.
Observation of Electrical Work Activities
1.
The Region III inspector observed five Class IE cables in the
main control room that lacked proper support. A similar condi-
tion was previously reported in IE Report 81-14.
Although
there was no evidence of damage to the subject cables, the
potential for damage was present.
In response to the inspector's
concern, the licensee took immediate action to have the cables
properly supported. The inspector has no further questions
regarding this matter at this time.
2.
During inspection of installed equipment, the Region III in-
spector observed that corrosion was present on cable lugs in
MCC NG04D. The licensee prepared Deficiency Report (DR) No.
2SD-4835 to properly identify and disposition this item. The
equipment has been turned over to Union Electric.
3.
The RIII inspector observed various electrical panels, cabinets,
and switchgears for proper terminations, cable support, cable
bend radius and cable separation. All items observed were in
accordance with applicable instructions, procedures and drawings.
4.
During a review of electrical cable installation activities,
the Region III inspector observed that SKV cable was being
separated by one cable diameter in accordance with the project
drawings. Three tray sections were also inspected to verify
that all installed cables were properly identified as Class 1E
(i.e. , red, white, blue or yellow).
5.
During a review of electrical cable tray installation activities,
the Region III inspector observed that:
a.
In the reactor building at elevation 2030', cable trays
4U2A08 and 4U2A09 were only separated by 26" from trays
6J2B08 and 6J2B09. Further investigation indicated that

barriers would be installed in accordance with drawing
E-0R2411(Q), Revision 9 in order to meet separation re-
quirements.

b.
"In the control building at elevation 2000', an unidentified
insulated 8" pipe was identified within twelve inches of
cable tray 4J1C63'and within twenty-four inches of trays
4U1C62, 4GIC62 and 4B1H62.
It was also observed that 8"
pipe, number 2-EF-07-S-015 and 017, was within twelve
inches of cable trays 4U1C56 and 60 4U1C56 and 60 and
4G1C45 and 60.

Pending a review of the seismic design analysis for the pipe
supports, this item is unresolved (50-483/81-25-01).
B.
Observation of Electrical Procedures and Records
1.
The Region III inspector observed that Daniel Procedures WP-230,
" Field Fabrication and Installation of Instrumentation" and
QCP-230, " Inspection of Fabrication and Installation of Instru-
mentation" do not address the identification of instrument sens-
ing lines.
In a position letter regarding this subject, the NRC
states that instrument sensing lines shall be identified in
accordance with IEEE 279, paragraph 4.22.
The NRC position was
discussed at length with licensee representatives who acknowledged
the requirements. Presently, the installation of instrument sens-
ing lines is less than 5% complete at the Callaway site. This
item is open pending review of procedure revisions and the imple-
mentation thereof (50-483/81-25-02).

During a review of the cable pull records for the NG System,
the Region III inspector observed that the Cable Installation
Checklist was marked "NA" or " Hand Pull" under the checkpoint,
" Maximum pull tension applied to cable during pull".
This
condition was evident for'all Cable Code CO2 and JIP pull records
reviewed. Appendix B to Bechtel drawing No. E-01013(Q), Revision
5, lists the maximum pulling tension for Cable Code J1P as 32
pounds and 65 pounds for Cable Code CO2.
The following cable pull records were reviewed:
a.
INGB10AA, length - 90', pull started September 2, 1980
and completed December 17, 1980, Csble Code - CO2.
b.
INGG01DB, length - 190', pull started July 15, 1980 and
completed November 13, 1980, Cable Code - J1P.
c.
INGG015E, length - 210', pull started July 15, 1980 and
completed November 13, 1980, Cabte Code J1P.

INGG02AD, length - 210', pull started October 27, 1980
and completed November 7, 1980, Cable Code - J1P.
e.
INGR01AB, length - 264', pull started August 13, 1980 and
completed December 3, 1980, Cable Code - J1P.

In each instance, items a. thru e. above, the licensee failed
to verify that the maximum allowable cable pulling tension and
sidewall pressure, as specified by Appendix B to Bechtel drawing
No. E-01013(Q), had not been exceeded.
The Region III inspector informed the licensee that tnis failure
to execute an inspection program to verify conformance with the
documented instructions, procedures.and drawings is considered
an item of noncompliance with 10 CFR 50, Appendix B, Criterion
X as described in Appendix A of the report transmittal letter.
