SUMMARY
OF WEEKLY INFORMATION REPORT Week Ending i:ovember 27, 1981 Pilgrim Unit 1 Boston Edison Company is investigating the cause of gouges recently discovered in the torus inner shell during a refueling outage.
H. B. Robinson Unit 2 On November 20, while Robinson was operating at 55% power, a water leak of 1.5 GPM occurred in steam generator A.
CP&L initiated shutdown of the plant, and on the way down -there was an indication of leakage in steam generator C.
Both steam generators are being opened.
St. Lucie Unit 1 On November 23, an amendment to the St. Lucie Unit 1 operating license was issued authorizing an increase in power level from 2560 MWt to a stretch power level of 2700 MWt.
Extension of Public Comment Periods A Federal Register notice has been issued to extend the conuent period for the Material Control and Accounting (MC&A) Reform Amendments; and a Federal Register notiGe has been prepared to extend the comment period on the Proposed Safeguards Requirements for Nonpower Reactors.
McGuire Unit 1 McGuire Unit 1 is undergoing eddy current tests of selected tubes in the Westinghouse preheater-type steam generator. This test program was initiated after it was determined that serious tube wall wear had been experienced in similar steam generators of Ringhals 3 (Sweden) and Almaraz 1 (Spain). McGuire Unit 1 is the only other operating plant with the preheater-type steam generators. So far, the first generator revealed no unusual indications.

WEEKLY HIGHLIGHT OFFICE OF NUCLEAR REACTOR REGULATION WEEK ENDING NOVEMBER 27, 1981 Pilgrim I A Boston Edison Company inspection of the Pilgrim I torus inner.shell revealed I
a number of gouges (7) in the torus inner shell. These gouges were identified.
after preparation of the surface for painting by sandblasting.
The plant was in a refueling outage with the torus drained for modifications at the time.
The gouges identified range in-depth from approximately 0.200 inches to 0.600 inches, (w90% wall thickness) and from about 1" to.about 4" long.
Gouge width was typically about 1-lk".
A Licensee Event Report was issued by the licensee on 1hl3/81.
The licensee is still investigating in an effort to identify the cause of this condition and detennine necessary corrective-actions.
ST. LUCIE' UNIT 1 i
On November 23, 1981 an. amendment to the St. Lucie Unit' I operating '
license was issued authorizing an increase in power level from 2560 MWt to a stretch power. level of 2700 MWt.
Significant issues reviewed include accident analyses affected by TMI-related design.and operation l
changes, and the use of statistical combination of uncertainties in the l
calculation of safety setpoints. No changes to the environmental Technical Specifications were involved.

-0FFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending November 27, 1981 Meeting with Department of Energy DOE reported that it is continuing research and development related to both wet and dry storage of spent fuels including in-pool rod consolida-tion with storage of consolidated assemblies and dry cask storage.
Ef-forts are directed toward providing technical support to utilities in meeting spent fuel storage capacity shortfalls.

Gilbert Associates, Inc. has been considering a dry spent fuel storage vault design for an ISFSI.

Extension of Comment Period for Proposed Safeguards Requirements for Nonpower Reactors A Federal Register notice has been prepared requesting an extension of the comment period regarding the Safeguards Requirements for Nonpower Reactor Facilities Authorized to Possess Formula Quantities of SSNM. This is in response to requests from interested parties. This will result in comments being due on February 15, 1982.

EN-81-43 Dayton X-Ray Company - A Notice of Violation and an Order to Show Cause was issued to subject licensee on November 27, 1981. The Order requests Dayton X-Ray Company to show cause why the license should not be revoked.
This action is based on an inability by the licensee to conduct its industrial radiography program in accordance with NRC requirements, and an unwillingness by the licensee over a period of almost six years to assure that radiographic procedures were performed only by authorized personnel. Significant violations concern instances where the licensee pennitted unauthorized individuals to perform radiography even 'though the licensee was specifically informed by the NRC not to do so, and a violation of_ an amendment to the license which allowed only one individual to perform radiographic operations.

EN-81-44 Carolina Power and Light Company (H. B. Robinson Unit 2) - A Notice of Violation and Proposed Imposition of Civil Penalti.es in the amount of $50,000 will be issued to subject licensee on or about December 1,1981. This action is based on alleged violations involving, (1) failure to follow specific established procedures for steam generator maintenance work, (2) the perfomance of inadequate radiation surveys resulting in an individual roceiving a radiation dose in excess of NRC regulatory limits and, (3) the use of an unqualified radiation control technician.which contributed to the excessive exposure.

Report from Region II:
On November 24, McGuire 1 was restarted after an eight day shutdown to perform eddy current examination of selected tubes in the Westinghouse
~
preheater-type steam generators.
Tubes examined in the first generator revealed no unusual indications. McGuire 1 (4 loops,1180 MWe) has not exceeded 50% full power and to date, has appr,ximately 100 effective full power days exposure.
The licensee plans-to examine additional tubes after startup tests'at 75% and 100% power are completed.
This test program was initiated after it was determined that serious tube wall wear had been experienced in similar steam generators of Ringhals 3 (Sweden, 3 loops, 900 MWe) and Almaraz 1 (Spain, 3 loop, 930 MWe). Tube wall wear is apprently due to flow induced vibration causing wear between tubes and flow baffles. McGuire 1, Ringhals 3, and Almaraz 1 are, to our knowledge, the only operating plants with the preheater-type steam generators.

ORNL' Blowdown Heat Transfer An analysis of a double-ended cold leg break simulation (THTF Test 3.05.5B) has been completed by ORNL. A draft has been submitted for RES review. A comparison of time to departure from nucleate boiling (DNB) data from a 15 x 15 bundle and a 17 x 17 bundle did not reveal differences that could be attributed to geometry.

LOFT Fuel replacement procedures are continuing this week to install new instruments in the LOFT downcomer and the pressurized F1 bundle in December. The peripheral fuel has been visuallyand photographically examined in place. No fuel damage was detected.
The scheduling of tests L9-3 (ATWS) and L6-6 (Boron Dilution) is now underway. The test sequence will be set to optimize plant turn-around.
Last week the facility was toured by Mr. Henoch of Mr. Brewer's staff at DOE Headquarters. Mr. Henoch is responsible for international affairs under Mr. Brewer.

Seiniscale Conversion to the intern diate break configuration is in progress following completion of the natural convection test series. The NRR requested test S-UT-8 to further evaluate the Westinghouse liquid level instrument will be conducted during the conversion, probably the week of November 30, 1981. An " add-on" test at the end of UT-8 will be performed (also at NRR request) to study pump restart with a voided primary loop. This will destroy the Semiscale pump seals, but if the resulting leak is small, the desired data will be obtained. Seal repair will be accomplished during the planned hardware modifications without impact on the program schedule.
Severe Accident Assessment Branch 1.
Hydrogen Burn Equipment Survival ~ Program Meeting Sandia National Laboratory made a presentation Wednesday, November 18, 1981 on the status and progress of the Hydrogen Burn Equipment Survival Program to members of the Chemical Engineering Branch, Equipment Qualification Branch and Containment Systems Branch of NRR and the Severe Accident Assessment Branch of RES. SNL discussed the development of thermal models and experi-ments to assess the survivability of safety related ecuipment during possible accident scenarios in which combustible concentrations of hydrogen were re-f leased to the containment. Also they discussed the amount of information that will be available for the Commission during FY 82 on this subject.

Managemant Meeting on R&D Needs Associated with Severe Accidents A meeting among DOE, NRC, and industry representatives on R&D requirements related to severe accident considerations was held on November 20, 1981, at j
DOE Headquarters, Germantown, MD. The meeting covered the following subjects:
objectives of rulemaking,. scope of rulemaking - industry views, NRC views and update on R&D requirements. A followup meetino is scheduled for mid-to-late January 1982.

The request will be forwarde'd to the Commission in the near future.
tDOE plans.to approve the request on the basis that it meets all statutory requirements and approval would be consistent with the gelaral guidelines -
provided in the President's statement of July 16.
Reprocessing of Reduced Enrichment Fuels from Research Reactors' In connection with two proposed HEU fuel exports to France for.use in research reactors, IP has-been requested by Chairman Palladino to prepare a report on the significance of and the prospects for reso,1ving the issues of reprocessing fuels containing silicide from research reactors in connection with the RERTR program.
IP staff notes that DOE has taken no official position on the acceptance for reprocessing of low enriched fuels containing_ silicon or materials other than aluminide, oxide, or zirconium hydride.
t this time, there are obstacles to the reprocessing of silicide materials which are technological institutional, and economic in nature.
Several more years of R&D by Savannah River will be required before they can make a judgment on the feasibility of r'ecovering enriched fuels from the spent silicide material.
Consultations with Mexico

l NRC, on November 25, was invited by the State Department to join the U.S. delegation j
to the U.S.-Mexico consultatiuns on nuclear cooperation, now scheduled to take l
place December 2-4 in Mexico City, because a discussion of the " expansion /
enhancement" of the NRC-CNSNS exchange and cooperation arrangement has become an agenda item.
J.D. Lafleur, Jr., IP Deputy Director, will represent NRC on the team, which will be headed by'K. Davis of DOE and J. Malone of State.
IP expects to address (1) the expansion of the present arrangement (now limited to the two Laguna Verde units) to accommodate Mexico's new reactors; (2) better use of staff training opportunities; and (3) closer contunication and cooperation during nuclear emergencies once the Laguna Verde reactors become operational (in 1984 and 1985, l
respectively).

On November 24, 1981 Mr. William B. Menczer, RSLO, Region III, was in Rosemont, Illinois attending a meeting of the Midwest Compact Group.
Governors' representatives from the States of Illinois, Ohio, Wisconsin, Kentucky, Delaware, Indiana, Virginia, Minnesota Iowa, Maryland, Kansas, Missouri and Michigan were present.
Discussions centered around the site selection process, development of the Compact Waste Management plan, composition of the compact language and the legislative approval process.
On November 25, 1981 Mr. Menczer was in Columbus, Ohio meeting with senior State officials from the Ohio Health Department, Ohio EPA, Ohio Department of Energy, Public Utility Comission, Governor's Office.

Zimmer Nuclear Station investigation report and the NRC's escalated j
enforcement action taken.
On November 23 and 24 Jim Montgomery, RSLO, Region IV, represented the NRC at a Low Level Radioactive Waste Conference with Rocky Mountain j
States in Denver, Colorado. The purpose of the meeting was to brief invited State legislators, governor representatives and interested l
industry representatives on the Low Level Radioactive Waste Compact i
i recently drafted by the States of Arizona, Colorado, Nevada, New Mexico, l
Utah and Wyoming who form the Rocxy Mountain Low Level Radioactive i
Waste Compact.
